Passport info for DS156

Timeline

Hi guys, I was filling out the DS156 form for my fiance and it is asking for Passport information but she already had her medical and they took her passport. I'm not sure if she has a copy of the passport. What can I do to get the passport info?

I read the same kind of problem before. I am not sure if I remember it well, but try to check the form that she filled up from SLEC and see if her passport number is written there.

It's in the copy of vaccination record the St. Lukes gave her. The passport number was written there.1130839m13ffzv93h.gif

Thank you so much, so that has the dates the passport was issued also?

No, it's not written there when it was issued there. Only the passport number. Doesn't she has any photo copies of her passport? Maybe she kept any of that. Or maybe she still kept the receipt that she paid at DFA.

No, it's not written there when it was issued there. Only the passport number. Doesn't she has any photo copies of her passport? Maybe she kept any of that. Or maybe she still kept the receipt that she paid at DFA.

Or better yet, call SLEC and ask for help/favor since they are the one who hold her passport at present. I hope they will be considerate to entertain your concern. But you can always try and see.

How long it your passport valid for? 10 years?

It is valid for 5 years. Do you already have the exact date when it was issued?

It's either Feb or March 2007

You need to get the exact date of when it was issued and when it will expire to be filled-up on DS-156. What other options are left for you guys to get her passport info?

Mine is 5 years. If you knew the date it was issued it would be the date it will be expired too, 5 years after it was issued.
