I could be wrong, but I thought this paper was more for use of fiance visa or for people who married under "religious" ceremonies that aren't recognized by the US? I know someone here will correct me if I'm wrong (hehe love VJ ) but if you married in Tunisia, the US recognizes that marriage certificate as being legitimate, legal, etc.... Are you using this as some kind of extra evidence of your relationship? I'm not sure if it would really be necessary. I had a friend who did use affidavits with her petition, but she was only engaged and filing 1 129 F and not CR 1 ... Hmmmm... I'm going to go research this and see what I can find - And look to see if I can find a standard affidavit form. However, as you already stated - It just needs to include the full name and address of the person who is signing for you - what their relationship is to your or your wife - and then a basic information regarding what they know about you ... "How you met" or something like that.

Hmmmm...
