Hi everyone,

I'm about ready to fill up the form DS-156 but i noticed i need my passport number. I had my medical exam April 1-2 and as we all know St. Luke keeps the passport as they will be the one to forward it along with the medical results to US embassy. I don't know my passport number off hand. :wacko: Any idea how I can get/obtain my passport number. Or would it be the same with my old passport number?

im not sure if its the same (i believe its not if i can remember it correctly)... dont you have a photocopy of ur passport for safety reason? or probably you can find some docs that has your passport# written on it... i used to practice a bunch of ds-forms before making it final so i have a lot of old extra docs that has my info on it... i hope you'll find ways too... good luck and God bless!!!

4/16/2010- signed and delivered

4/29/2010- check cleared

I didn't use my new passport except when i filled up the forms during my medical exam and i didn't make any photocopy of it too :bonk: I didn't realize i need it for my DS forms :crying: I'm screwed hehehe I'm not sure if calling St. Luke's will do me any good waaaa

Agreed to Ronmay,

though if you don't have any copy of it, then i should suggest try to go to St. Lukes and try to talk to the person in-charge with the records that it is important for you to get to know your passport number, however, make sure to bring at least 2 valid ids or any proof of identity that it is you, and a receipt of your payment at St. Lukes.

There is no harm in trying na makiusap if there is really no choice or no way left.. :)

This is just my idea if that will happen to me, though i memorized my passport number already. lol

i hope they will honor it and that i hope they havent forward your passport in the embassy already which they do asap... if they do, lets just pray they photocopy all the docs for their own records and copies... good luck and God bless!!!

Really appreciate your fast replies :thumbs: I'll try to go there tomorrow and hopefully I can get my passport number. :innocent: My interview will be on the 23rd and I need to print those forms within this week so I won't cram ;) I'm worried though what if they can't help me :crying:

Well, if they disagree to entertain your concern, then i suggest "beg" , lol.. :D Anyway, since most of them are busy , i think ready yourself to have a very nice convincing power to let them find and look the record for you. i hope you find your way tomorrow.. It's important to put your passport number in filling the Ds156 form.

HI JOYCE, DON'T FRET. DID THE SLEC STAFF GIVE YOU A VACCINATION DOCUMENTATION WORKSHEET TOGETHER WITH THE X-RAY? CHECK OUT THE FORM AND YOU WILL FIND YOUR PASSPORT NUMBER AT THE TOP PORTION RIGHT BESIDE YOUR BIRTHDATE AND YOUR ALIEN CASE NUMBER. IT'S RIGHT THERE. I'd also suggest filling out all forms ahead of time, except the signatures and date. Then you'll be prepared.

You're my angel :yes: Yup, I checked my Vaccination Documentation Worksheet and there it is :dance: wooohooo problem solved. Thanks a million. I was sooooo stressed out last night and worried sick about this hehehe. Now i'm ready to fill up the forms and for the interview.
