Hello,

My wife is trying to show in her Ukrainian passport, that her name changed when she married me last year in the USA. This is so her green card married name matches her Ukrainian passport when we travel (we had problems in Odessa because her passport is in her maiden family name) . We were told today from the US Immigration & Natu that the Ukrainian Consulate in the USA (Chicago for us) can put an Addendum in her Ukrainian passport showing her name changed when she got married. Not that the passport is replaced, only add a page document to it.

Does anyone know anything about this "Addendum" ??

This is what we did for my wife's Russian passport. I assume it would be almost the same for the Ukrainian passport. Did you check Ukrainian embassy website? The Russian embassy website explained what we needed to do, but I remember my wife calling and speaking to someone about it. In our case I think we sent a copy of the marriage certificate along with her passport and a money order for the correct amount. The Russian embassy was VERY SLOW and it was about six months until we got the passport back.

Inside of her passport there is a stamp with a note written in that in the US she goes by this married name. Right after she got the passport back we traveled to Russia and there was a question why her ticket was in her married name and her passport was in a different (maiden) name, but when she showed them the stamp (I think this was in the Atlanta airport) everything was fine.

I don't know how it works in Ukraine but in Spain they can put a stamp on the passport stating that the passport holder is married as well as adding the husband's name. They can also write this info on a page of the passport. Once again i don't know how it works in Ukraine but I'm sure if you explain the case at the embassy they'll be able to help you out.

Adjustment of Status

02-29-08 Sent I-130, AOS, EAD, AP (Via FedEx) to Chicago

03-03-08 Package arrived in Chicago

03-07-08 All Cheques cashed!!

03-10-08 NOAs for I-130, I-485, I-765 and I-131 received

03-16-08 NOAs for I-130, I-485, I-765 and I-131 touched

03-27-08 Biometrics Appointment at 2pm (Notice date: 03.11.08)

04-30-08 Received EAD "card production ordered" & AP Approval notices from CRIS (YESSSS!!!!!)

05-06-08 I-765 touched and AP received in the mail!

05-08-08 Received I-765 "approval notice sent" e-mail from CRIS

05-09-08 EAD received in the mail (YIIPPEEYY!!!)

08-22-08 Notice of Action for initial interview received (10-08-08)

10-08-08 AOS Interview in Los Angeles: APPROVED!!!!

10-09-08 I-485 touched! (Card Production Ordered)

10-15-08 NOAs for I-130 (approval notice) and I-485 (welcome letter) received

10-22-08 Green Card finally arrived!!!

Removing of Conditions

07-14-10 Sent I-751 (Via USPS next day delivery) to California Service Center

07-22-10 Received NOA 1

07-30-10 Received NOA 2 (Biometrics appointment on 08-11-10)

08-31-10 NOA for Removal of Conditions approved

09-02-10 Approval letter received

09-03-10 Green Card arrived!!!
