Hi

i am filing for AOS, i have completed :

i-765

i-485

i-1135

g-325a

i-864

pls can someone advice on i693. i have completed a medical in UK london. also done my vaccincations before i left from my own doctor and have a letter showing this. Do i still need a civl sergeon? also does anyone know any civil sergens in New jersey?

i have now found my ds3025 but very confused in undestanding it.

in the medical they advised me to take three vaccines, which i took by my own local pharmacy and have a letter to confirm this.

looking at the ds3025 it states i dont need to take any??

there is like a (k1) wriiten on "vaccine given by panel site" and also at botton there is a tick on "US vaccination requirments complete"

i am so confused?

If you attended the medical and they told you to get three shots, then they are not written on your DS-3025. You got them after the medical. Right? That means your DS-3025 is incomplete.

The USCIS will not accept a letter from a pharmacy. They must see all shots with dates on the DS-3025 or on the I-693.

MMR- two shots, or one recent shot and the waiver column marked insufficient time interval

Tdap or Td with a date within the last 10 years (2005 or sooner)

Varicella-- either a tick by the shot with a date or a tick by Varicella History

Flu shot with a date.

If all those are not on your DS-3025, then go get them recorded on an I-693 by a Civil Surgeon.

i am so certain the Dr at knightbridge advised i can take them from my Dr and get proof. Which i did.

so will i need to get the Vaccinations done twice? as i already have just taken the three vaccines required last month?

sorry my last question, will i need to retake the medical again as you stating my DS3025 is incomplete? pls advice which part of the i693 will i need to fill out?

The nurse at Knightsbridge has no clue how AOS works in US and gives out false information. If you had gotten them from your doctor before the medical, you would be clear for AOS because you would be signed off on an official immigration immunization form, the DS-3025.

You may take your proof from the UK...any and all records of immunizations...to a Civil Surgeon so he can put them on the official form and sign off. You do not require a new medical exam, but many Civil Surgeons will say you do. Don't believe that. Or they refuse to do the immunization part without doing a full exam. Call down the list of every one near you, asking prices to fill out only the immunization section. If they say you need an exam, hang up and call the next.

Read the instructions for the I-693 and you will see that it says K1s do not need another exam unless a year has passed since their visa exam. It will tell you which part to fill out.

please dont mention Knightsbridge, lol i have had two medicals done and nothing but issues with them.

i have spoken to a Dr Noel in New york just now and he stated $50 to transfer my vaccine info onto i693.

That's a reasonable price and you are lucky to find him. Some people call every CS in their state and all want $200+ and a full medical.
