I dont have a sample, but wanted to say I didnt send 1 & it was fine, others also didnt send 1, some do some dont,

What is a tax release form? I haven't heard of one of these.

Edited to say the "save as" thing isn't working properly. Hopefully this works... Don't forget to add your barcode and case number in the appropriate place, and replace "where?" with the name of your embassy.
