Packet 3 includes:
Completion of ds230 ( part 1 only )
Original Birth certificate
Original divorce decree ( if applied )
Original CENOMAR or ADVISORY OF MARRIAGE
Copy of residence card ( as Alien card no longer exists )
2 ( 2"x2" or 50cmx50cm ) pictures with white background
Copy of passport ( biographic page )
Email address ( a small piece of paper )
Letter head from USEM-tokyo ( put this on top of all the docs )
+ send all these to visa consular unit using letter pack available at any post offices for ¥ 500.
USEM -Tokyo will send you an email ( packet 4 ) once they reviewed and approved all submitted docs. Packet 4 requires you to schedule your interview.
REQUIREMENTS FOR INTERVIEW:
Confirmation page of completed ds 150 ( with barcode ). Once completed online needs to pay visa fee so you can schedule your interview )
Passport
Receipt of payment for k1 visa ( payment at post office or any banks for ¥ 31600 )
Affidavit of support ( fiance's latest w2, pay stubs, certificate of employment, bank statements )
Medical certificate ( took the medical examinations after we get our case number because it'll takes a week to release )
Police clearances ( requires from places you have lived for more than 6 months. I took my japanese police certificate ahead too because it'll takes more than 2 weeks to release )
1 passport size picture ( same as submitted on packet 3 )
Evidence of relationships ( pictures together , chat logs )
+ No need to submit civil documents ( birth certificate , divorce decree ) as these are already included in packet 3
* I strongly suggest to apply for CENOMAR or advisory of marriage and NBI once you get your case number as these will take too long to release. I went to Philippine Embassy- tokyo for NBI application and fingerprintings only. Send approved application to Philippines ( needs a family or immediate relatives to submit to any NBI satellite branches ). Ask your family or relatives to get your CENOMAR and birthcertificate alongwith because they will send you back your NBI certificate as well. You can apply online for these civil docs but will take more than 2 months to receive.
** I also advise to take medical examinations and police certificates once case number is given for speedy processing
