All my hubby did was email NVC with subject line: MNL123456xxx DS3032
He followed the instructions http://www.visajourney.com/wiki/index.php/Email_DS-3032_form and received acceptance email from NVC in 4 days.
There's no need to send mail hardcopy.

It needs to be US format passport photo...meaning 2x2.
I had the same issue when my hubby had his passport pic taken at SM in Manila. Philippine version of a passport size pic is different from that of the US. We had to have it retaken and resized to a 2x2.
