Hi Maria, seems like there will be a lot of things that you need to change on your form so I will suggest to just fill out another DS156 online. It will give you another serial number though. You could still bring the form that your hubby filled out if ever they ask you why you changed it at least you have proof coz I dunno if they have a system to track serial numbers
