I believe the DS-156 is an online form. Here is the link to it. You fill it in then print. You cannot save it.

sorry for the late response. When you got to banco popular just pay the $140 and you will have to also pay $210 at the Embassy. Make sure you get your receipts.

The embassy in Santo Domingo does not issue a Packet 3 (P3). It only issue Packet 4 (P4). The instructions here http://santodomingo.usembassy.gov/k1-k2_instructions.html

are basically the same as the instructions you receive with P4.

Check my timeline to see how much time passed. But I think it was 2 months from when I got approved to the date of the interview.
