Read carefully the instructions provided in the link below:
    http://travel.state.gov/content/visas/english/immigrate/immigrant-process/interview/prepare/medical-examination.html
From the above link, follow step 1 and 2. After you get a PDF (based on your mom's country of residence), see which designated medical facility is close to her residence. Call them and set an appointment. Ask them what documents to bring. Most probably they need following docs:
    1. Copy of Visa Interview Letter
    2. Copy of Biographic info. pages of the passport
    3. List of any medication
    4. Two to Three US Passport size pictures.
Above requirement may vary depend on the country of the residence.
    Good luck!!

Lol Quyana! You hit the nail on the head. I've been super busy. Mom's visa was printed yesterday so she'll probably receive it next week.
I'm still waiting on MIL's papers to send in.

They treat each parent as a separate case. That is why on your AOS, you will list your family members plus ONE parent on each AOS. i.e. if you have a family of you, your wife and your son and one parent, you will write a total of 4 for your household size.

Its slightly reassuring to see that when I log in with my Dad's old case number, a message pops up saying that it has been changed to the London one Puts my mind slightly more at ease.

Termination message is a fluke. Don't worry about it. Just wait a couple more days. A lot of people here got the same message.
If you are not the beneficiary and you fill out any form, you have to list yourself as the person who assisted the beneficiary in filling out the form. There's no harm in that. NVC knows perfectly well that our parents often have no English on them and/or no internet. Lying to NVC, on the other hand, is not a good idea.

When I got the bar code sheets I got two different address also but since I had no intention of paying two shipping fees I got two yellow envelopes wrote the addresses according to the barcode sheets for each then I put them both in one USPS express mail envelope and sent them to the address with ATTN: DR. They are in the same building but different suites so I'm sure someone can walk it over to the other suite. I did this twice and had no issue.

I think that he is a third-party agent, because you are the petitioner and he is definitely not the principal applicant.

No, you don't need her signature in ink. But remember to fill out the space for "who helped you prepare this form". I'm assuming you will do that?
The biographic data page of the passport should double in English. I've never seen an Armenian passport though:) Russian ones do.
You can make all the photocopies yourself and you also can translate everything yourself if you are able and certify the translation yourself too. Everything that is not in English has to be translated.
You also have to submit your birth certificate and your marriage certificate (if any).
You need your mom's marriage and divorce/death of spouse/whatever applicable documents too.

Three days. Although the case number could have been assigned before I got through to NVC and received it, possibly the Friday before. Three business days nevertheless.

Yes. You have to use it. If your papers get there before the payment clears, they'll just hold them. But that's unlikely, the payment clears in 2 business days.

I got my tax transcripts today and we have always filed married filing jointly. My question is this: how does the NVC determine what was my income and what was my husbands? Do I need to include W2's as well for the past three years?
