  1. :thumbs: Same here.I used my husband's family name to all of the forms and other paperworks like when I applied for SSN,State ID,bank account.And my middle name now is my family or last name when I was single.

    I suggest its better to use your husband's family name in all of the paperworks especially in your greencard so it will match your passport once you renew it or amend it with your married name para wala maging problema in the future.

  2. My wife's DS-3025 had both of those boxes checked too. TD and Varicella were circled wth TD being checked as Not routinely available. The Varicella has a vaccine date with Insufficient time interval checked. The DS-3025 was sufficient for my wife's AOS, but consulting someone with more experience in vaccines would be a good idea.

    Thanks, we called Dr. Arnold's office yesterday and we spoken to Tammy and told her what vaccination I had in the Philippines. And she advice us not to take the 2 vaccines w/o her advice until July 20. Coz the scenario was I took that 2 vaccines in our country already but at the right side of the form there is insufficient time interval, my perception here is I still need to take it here upon my arrival in US. But she told me not yet, wait for her advice if I need to or not necessarily. And that's it we follow her instruction, we just sent today the copy of my DS-3025 and part 1 of I-693.

    Hope everything will be OK w/o the 2 addt'l vaccines.

    I am intruder here in June filers, but I want some help here with regards in I-693. Reading the forums, there are civil surgeons asking to do the whole medical examination process. I live in New Hampshire and the nearest Civil Surgeon here is in Nashua & Colebrook. Does anyone here done his/her vaccination near at this area? Because I just want to know if they can just do the vaccination only not the whole medical examination processing but still transcribe the vaccination to I-693.

    Your K-1 medical from St. Luke's is good for 1 year. As long as you are filing the AOS before that year has expired, there is no need to have another medical in order to file AOS. Some civil surgeons will try and get you to take the entire physical again....either because they want to make more money on you or they are misinformed. Explain to them politely that it's not required because you have already had a physical for your K-1 visa within the past year. If they refuse to do the vaccinations without the physical, then I would look for another civil surgeon. If you look around, you can usually find a civil surgeon that will transfer the vaccinations for a small fee. I think I paid $25 for my wife a couple years ago to have it transferred.

    If, however, you're filing your AOS more than one year following the exam at St. Luke's, then you will need another complete medical.

    We called Dr. Arnold to transcribe my vaccination. I will take my vaccines at the health center or hospital near in our area.

  4. Hello, there! My husband and I went to the Civil Service Surgeon here in Allen park , MI., just this morning. I had my 2 vaccines(Hep.A, and Meningococall). I was just worrying now because the nurse assisted me just give me a sealed envelop of my vaccines.,labeled at the back Immigration documents, do not open. It is just a Vaccine History transferred from a written record, this is the page 3 of I-693. They said this is the only paper allowed to give me because I just had my vaccines only....In the instruction of the I-693, it was stated that I need the Part 1, and Part 5. We paid 200$ for the 2 shots. I was insisting to the nurse that the oher part needs to be included in the sealed envelop, but she said that`s the only thing they are doing for the vaccine shots for the immigrations!

    Would this be okay?what if the Immigrations look for the other part of the Medical/Vaccination report? the vaccine report was already sealed and it is in my hands now. Please share us your insight for those who have been finished for their Interview.

    I am sorry but I am not here to answer that but to ask/inquire if you already passed your I-485 or the application of change of status..meaning that you haven't receive any RFE yet am I right? You just done this just in case that they will gonna ask for the I-693?

    Because I am in planning to submit the application this month but confuse if I will go to the civil surgeon for my additional vaccines & for them to transcribe my DS-3025 or just submit my DS-3025 and just wait for the RFE?

  6. This is not entirely true.

    If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. (this is what I had in mind) The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

    You may still get a RFE for an I-693, but many VJers have been approved with the DS-3025. The vaccines needed depend upon age of the benficiary. Maybe someone will post the needed vaccines here.

    Actually mine is not yet complete I believe coz the result in my DS-3025 was -vaccine history incomplete: applicant maybe eligible for blanket waivers because vaccination not medically appropriate.. Based on this I think I will be needing the vaccine for TDAP & Human papillomavirus, which I already taken in the Philippines but on the right side of the form they check the insufficient time interval, please correct me if I am wrong that I should need to take that here in USA to complete the vaccination, then I am all set.

    I am thinking to go to civil surgeon then bring my DS-3025 & X-ray cd, get the 2 additional vaccines, i will just let them know that I already finished my medical examination in the Philippines, then they can transcribe my DS-3025, the additional vaccination to my I-693. I am just worried if the doctor will insist that I should undergo another whole processing of medical examination, which will not be good in money side :).

    We are already gathering/preparing for our papers for AOS. I just want to clear this up if should I still need to undergo medical examination or vaccination if I already had one in the Philippines & they already issued the DS-325.

    Q: Why are K non-immigrant visa holders asked to include a vaccination supplement when filing Form I-485 to adjust status?

    Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include form I-693- part 1, part 2-vaccination section, and part 5 in the application.

  8. hello..i am from fredericksburg,virginia..Nope,am not yet finish my I-693 probably contact the civil surgeon next week..Filling the AOS would be the last week of july!!!Anyways,how many vaccine they will be given to me?thank you..

    I just have a question with this, are still going to have vaccinations here? We already had in the Philippines right? So, All we need to submit is the form that they gave to us DS-3025. Please correct me if I am wrong..

    Thanks for that info. I was about to ask almost the same kind of questions. Now I am preparing some documents that we need to my change of status here. I already got married and I am planning to get SSN soon, but it should be on my new surname right? coz I am already married.

    You don't have to be married to get a Social Security number. Get that in your maiden name about 10 days - 2 weeks after arriving. Change your name on it later after your green card or EAD card. It confuses the situation to try to use married name because SS people have to find your record of arrival at POE and that would be under your unmarried name.

    Get a bank account together before he leaves as proof of joint things.

    Put the aunt's address on you AOS things so you will easily get the mail from them. If he is deployed, then they will understand you can't be living together. He will need to fill out and sign the I-864 Affidavit of Support and provide evidence of his income. All the rest of the forms are yours to fill out and sign. If you get an interview, I don't know how he will attend, but I'm sure others will know and respond. Maybe you will be lucky and have your case transferred to California Service Center and won't have an interview.

    I saw your time line here and we are also in December filers. Have your fiancee already received a letter from the embassy?

    Me & my fiancee are actually preparing some documents for the interview. I just want to make sure if we are doing the same things as you do guys.

    I hope there is a forum for us, NOA2 approved December filer's to get help from each other. :) That will be nice
