Assuming not COVID:
DOS does not determine if entry into the US is allowed. The issue a VISA that allows you to SEEK entry into the US.
If you leave the US to Canada and your I-94 has not expired you are not given a new I-94.

If you leave the US to Canada and your I-94 has expired expect that you will have issues as a B2 tourist visa being used for another visit if a significant amount of time has not transpired between exit and attempted re-entry.
