No they online require a scanned copy. She still has her card but it expires in a few months. Her I-90 receipt form (which extends her current card expiration 3 years) last name doesn't match the name on her current green card.

Her married name was on her conditional and also her current green card.

The problem is that her maiden name is on the I-90 receipt which extends her current green card expiration date by 3 years. Processing times are over two years..

Thank you for the response olduser. We do not have the green card yet though. Just started the application process and found that it is in her maiden name through the forms they sent and USCIS website.

Hello All,
My wife recently filed her I-90 Application. To our suprise, her maiden name showed up on the application online instead her married legal name. I know for a fact she used her married name on the application.
What would be our next course of action?
My guess would be to file another I-90 application and in part 2 tick the 2.d. box (My existing card has incorrect data because of Department of Homeland Security (DHS) error. (Attach your existing card with incorrect data along with this application.) Then I would state the error in Part 8 additional information including the WAC# of the already submitted application.Thoughts?

Yes she plans to.
We resolved issue so good to go now. She never received her I-90 receipt in the mail but she found it on the USCIS website. The receipt has her correct last name along with the 36 month extension on her current green card.
I am not sure why the USCIS website and the I-797 biometrics form had her maiden name.
