    Thanks Appleblossom, I have contacted the citymed multiple times and finally they have booked my medical appointments. I think the staff there are confusing. NVC also confirmed there is no other medical form needs .

    Hi guys, I have received my interview confirmation email from NVC , my interview will be in early June 2025 in Auckland US consulate. The next step is to complete medical exam at citymed. However the citymed advised me that the medical form on the https://travel.state.gov/content/travel/en/us-visas/Supplements/Supplements_by_Post/ACK-Auckland.html#medical_exam_instructions is not completed. It should be approx 16 pages long. Citymed did not proceed my medical examination booking until I have the completed form. They said the medical form on the website is only the front page. I have been contacting Auckland US consulate but no response until now. Does anyone have any idea or having the form? Much appreciated for your help.
