Ok a follow up on this as promised. Rainier Occupational and Family Medical Center 1400 South Jackson Street Seattle WA They are listed on the USCIS find a civil surgeon page. They charge $90 for transcription. You bring proof of all your shots. You put in the info you need to in the form, they will fill out their part for section 10 and give you it in a sealed envelope.

Update 2: The site was not rendering correctly and that was where the confusion was coming form. I want straight from Indian site to VFS global. I was wondering why it was looking so weird. I logged out before completing my application. When I logged back in again it rendered correctly, was less confusing. Try doing that (though you will lose all the data you have put in). (to mod I tried to edit original comment but it said to late, hense the multiple comment reply)

Update: I figured it out. So there first part of the Visa or other type of application is filled out in field above. So for example your Application Id is: USASXXXXXXXX Just put the XXXXXXXX in the second field. (of course replace the XXXXXXXX with whatever the number/letters are) The thing is I put just the second part in and it still didnt seem to work, so I took at the first digit hit backspace, then put in back in again and it worked. so might have to play around with it.

Just going to add to this. This is for a Visa, I am putting in the Application Id. I can't find anything different to put in there. At first I was pasting it, but I also typed it in. Nothing seems to work.

Did you end up getting it changed to the local office? Or did you have to fly back, get a hotel and all of that?

Thanks for the response. To clarify you got the full medical here (not the medical outside the country, and then just the vaccine part here)? You went to the interview and the doctor had not sent the medical report yet as you had not got the Hep B shot yet. They asked you about it and you told them what happened. Later you got the Hep B shot (or at least 1st in series?) and the doctor then sent the report that indicated that? Trying to determine if you went to the interview with no report OR they had sent the report but it showed that the Hep B had not been done yet? Dealing with a similar situation so trying to take best course of action, as will be unable to take one of the shots before the interview.

As has been mentioned. A letter is fine. Just follow the format that has been listed in this thread.

You have an American passport, not a criminal, just put visiting friends. Outside of say Yemen and a few other countries you will be fine. American LGB peeps travel all over the world.

I hope this works out for you. I think some people for sure should take the vax, but I do not think it should be a requirement. The vax mandate is being dropped as requirement by many universities and govt institutions in the country. Hopefully this will be the case for applicants.

Can you tell us what happened at the interview as you didnt have the Hep B shot before?

Yep I missed the the Hep B. Yes covid, in my mind I was thinking of one people probably had as a kid but don't have records for.

There are only 4 vax to check for an adult. Tdep MMR Influenza (if in season) Varicella (which is un needed if have had chiken pox) The medical assistant can check the records the patient provides, the doctor just has to sign off, like 5 mins. Their time is valuable but they are not brain surgeons. Two of the doctors I called actually answered their own phone. Will not pay $300 for a paperwork check. Will share with board when find someone who is reasonable.

Have called a couple of them. A lot of them went to voicemail. One didn't want to do just vax part. Other wanted to charge $300. Will be calling the rest this week. Will update the board here.

Making a post about Civil Surgeon fees in Seattle for 2023. Before making a new post I searched and it seems last post on this was over 10 years ago in 2012. Looking for a doctor who will do the transcription for section 10 for I-693, the vaccine part. As was the problem a decade ago, some of the doctors want to charge $250-$300 like they would for the full medical, when just need to the vax part. If anyone has had experience lately, please share.
