    Thank you for your reply. There isn't note about Covid-19 on my form. I asked the hospital and they said they cannot write about the covid-19 vaccine because it is not required to be proved for K1 visa.

    Thank you for your reply. I arrived in the US April 11 2022. I took the medical exam the end of January 2022.

    Hi all, I got Covid-19 vaccinations and completed DS-3025 in my home county for K1 Visa without any issues before I came to the US. However, there is no area on the form to mark that I received the vaccination. Do I need to go to physician and ask to fill out Part 10 Vaccination Record on Form I-693 or can I just provide my Covid-19 vaccination certificate from my government? I appreciate you time.
