I emailed the embassy and in this case, the petitioner name is an annotation and they said it is fine to travel. My name as the petitioner is correct everywhere else. The most important name is his name (beneficiary). I will keep you posted and thanks for the input! 

Interview was on Jan 17 and visa was issued and ready for pick up on Jan 28 (6 business days because Jan 20 was a holiday) 

My name as the petitioner is misspelled on the K1 visa in my fiance's passport that was just issued.  I have done multiple cases to let the consulate know and I keep getting a generic response that they are escalating the case.

Does it even matter? Will it impact future things like AOS or do I just ignore and move on since my name is correct on all other documents?

My fiance went for his medical exam at Servicios Consulares on Monday and he did not need any vaccinations for the interview. They did, however, say he will need a few more vaccinations when he comes to the US. Are these for AOS? It is also common to have to go back to pick up the medical results and then take them to the interview?
