  1. Other have posted that they have just taken regular vaccination records from a family doctor and had those transfered ... Good Luck

Thank you for all the advice.

I finally went to a civil surgeon in Virginia to fill out the vaccination supplement and had 2 shots last week...he said that mine were received too long ago...:s

What kind were recieved too long ago? That sounds kinda strange.

They are MMR, tetanus & diphtheria!

  2. I sent the AOS on 4/4 and it arrived in Chicago on 4/8. I am now waiting for the NOA, hope it will be here soon.

I will move to the west coast by the end of this month, hope we can find a place to live soon, so I can file for change of address and I will not miss anything!

I am really worried about this!

We have same dates so far... I am going to add you to my friend's list so we can follow each other's timeline.

    COOL!

  3. Hi,

I am new here. I am now collecting all the document for filing AOS.

I entered the US with my K-1 visa, and I had my medical exam in Sept 2006 in my homeland, the doctor just gave me a sealed envelope and it was given to the immigration officer during my entry to the US. I have nothing else from the doctor except a chest XRay in a sealed envelope.

I remember that 'vaccination exempted' was written on the instruction for my medical exam, when the consulate sent it to me and requested me to do that exam last year, what does that mean?

Could anyone please tell me if I need that vaccination supplement I-693A.

And how can I obtain a I-693A if I really need one? Please advice, thanks a lot!
