In Peru, we used Unilabs (unilabs.pe) for first trip and Pulso Salud (pulsosalud.com) for second trip. Both have various locations throughout Lima and I believe Unilabs does testing at the airport during certain hours but you will have to check as that may have changed.  The clinic has to be approved on the list of qualified testing centers. I have attached the file of clinics.

Thank you so much! I will look into that PDF.

The health declaration form is pretty basic and yes, form is required for all. If an individual has recovered from and/or tested positive for covid in the past 3 months, then they need to have a letter clearing for travel by a licensed healthcare provider or public health official.

This is (generally) the form below. We filled ours out in advance, but at check-in they made us fill out their version which was almost exactly the same.

    Hi!

Did you have to go to a specific clinic to get the COVID test? Or can it be any clinic?

does anyone have information on interviews for immediate family members for the i130, we were DQ'd April 6th but from our understanding with the shut down of the embassy and the government issues their only doing a few interviews. we tried emailing the embassy for a ball park time frame but it's a auto no reply response. Just want to get some kind of time frame as to when interview may occur. Any peru i130's out there ?

Did they get an interview date yet?

Yes. I have travelled twice since Jorge Chavez Intl opened back up. It's a bit of a pain but manageable. It was covid tests (pcr or antigen) within 72 hours of travel plus a health declaration form, mask (now I think it's double) and face shield throughout the airport. Once we were on the plane it was just a mask (though this may depend on the airline). Coming back to Peru was essentially the same steps...tests, masks (plus face shield upon return to Jorge Chavez), Peru health declaration. I think that was about it.

    Thanks for responding!

Do you need the health declaration form if you have never tested positive for COVID? I read on the CDC that you have to get that done if you can show proof that you received a positive test result.

If you plan to travel internationally, you will need to get tested no more than 3 days before you travel by air into the United States (US) and show your negative result to the airline before you board your flight, or be prepared to show documentation of recovery (proof of a recent positive viral test and a letter from your healthcare provider or a public health official stating that you were cleared to travel).

Can you get a test from any clinic or are there specific ones?

Thank you for your help! My parents are having their interview on July 2nd so any information is greatly appreciated.

Yes they usually would send an email after 2-3 months if all required documents have been reviewed, if not they will ask for the documents you are missing.

Nice!!! It’s been a year I think! My father is just waiting now for an appointment date, but I just submitted a missing document for my mother’s case. I am hoping they would still have the same appointment dates.

Yes, it's been a while since my case closed in NVC. It closed back on April 2020 so yeah a little bit more than a year. It's crazy how they notified me just yesterday about the interview date and their appointment is already in a week and a half. It gives them very little time to get their medical exams done.

Today 4/21/2021... I FINALLY RECEIVED MY MOTHER'S APPOINTMENT LETTER!!!!

If you are like me, waiting for months from NVC to schedule the appointment, please go their website and fill-out the Public Inquiry Form, ask for what is the status and they schedule you that appointment few days after!

OMGosh I'm going to try that. I started this journey since February of 2019 and already completed NVC but no interview date yet.

What did you do? I’ve been using my birth certificate for every paperworks I’ve done till I became a citizen. I don’t get it.

Where are you from? I'm originally from Peru and the exact same thing happened to me. When I was getting my residency I used that same birth certificate which was accepted. Now with my parents visa I used the same birth certificate and they also rejected it. What I had to do was have my parent go to RENIEC which is a government agency that seals birth certificates and marriage certificates for its citizens. Since my birth certificate and my parents certificate was issued in a very small town they were not recognized by the NVC. That is when they when to RENIEC to let them know about the problem which the agency stamped all the certificates with their seal. Plus I'm not sure if this was helpful but when they received those sealed documents they went to the office of international relations and they stamped another paper onto that which states that the document is valid to use outside of the country.

I hope this explanation was helpful if you have other questions please let me know.
