    1. How should i sign the form? under "signature" --- the signature appearing in my passport (with my maiden name) or a new signature with my married name (with this though, i still have no other documents with a new signature)...How did you sign yours????

  2. CONGRATULATIONS TO EVERYONE WHO GOT APPROVED AND GOT THEIR GREENCARD...

    we got married last nov 4 and now i am in the process of filling-out the forms and gathering documents for the AOS...now i need help for the following:

    1. Address - C/O

    2. What should i put in line "Current USCIS status" in Form 1-485?

    3. Were you inspected by a US immigration officer? (I handed to him the yellow envelope and asked me some questions before stamping my !-94).

    4. In what status did you last enter?

    5. Applying with you? Under list of husband/wife?

    Thanks for all your help and support...wish we could get this through smoothly...I will also file an EAD with this form...am i right that the total fee is $1010?

    You may want to use the sample form here in VJ as your pattern because that's what I did, but to answer your questions, here it is.

    1. Just you and your husband's address, don't worry about the C/O

    2. K1 Visa holder

    3. Yes

    4. K1 or K1 Visa holder

    5. You have to make this one clear because I looked for this on my own form and didn't find it

    I believe it's correct, $1010 including AP and EAD. Good luck!

  4. When is the timing to apply for a state ID and driver's license? Would it be better to apply it after the wedding (with the marriage certificate)?

    one more thing, what if somebody wants to hire me part-time in an ice cream shop after d wedding? am i allowed to? wont i get into trouble?

    Your I-94 allows you to work until it expires per SSA regulations (I have the link for this at home) , the trick however will be in finding an employer that will accept it. With the current focus on illegal immigration this will be even more difficult. Good Luck to you!

    We applied for a SS number 3 weeks after Novelyn arrived in the US and had the card a week later.

    I dont think I have the EAD stamp on my passport at POE. I receive my SSCard today though it says there that "valid for work only with DHS authorization". What does that mean? Is this what it meant by I can work till my I-94 expires?

    The employer may hire me part time coz he believes that i can work after getting married. However, what i am worried about is the legality..what if he reports my name for withholding tax purposes? etc.

  6. So what documents you showed guys to the civil surgeon that we had the physical exams and some bloodworks done in the Phils? All the medical reports except for the vaccination records and xray plates were forwarded to the immigration officer upon POE. None of us have the copies of the exams taken and medical results...I asked this since one civil surgeon insists to give me a physical exam and bloodtests since he wont be able to see a proof that I had it in the Phils. And one surgeon wants that the physical exam and bloodtests must also be done by a civil surgeon...we called lots of offices already although there is one office said that as long as we provide them proof that I had everything done already, then they will only do the transfer of the medical records.

    Please give me advice on the proper support and documents.

    We had the 3025 record from St Lukes and the records from the health dept. for the TD shot and TB skin test

    yup..got the same papers do but how did you convince the civil surgeon that you do not need to take another physical examination & blood tests (HIV,syphillis, etc)? As I am reading the form 3025, it only says something on the vaccine records and not on any physical exams/bloodtests.

Nobody knows for sure but we paid $50 for my vaccination supplement, reading the TB skin test that I had at the city health department and attesting that even though the result was positive (size of the red swelling on my skin was 15mm), is because of the BCG shots I got when I was a kid. You need to get the list of Civil Surgeons in your area and make some inquiries. Just remember, you only need the vaccination supplement, not the entire medical since you came here using K1. The only reason why you will need to redo the entire medical is if the last medical you had is past 12 months.

    Yeah.. don't get the full physical! In my case, although I said it again and again that I only wanted the supplement, they almost gave me a full physical. Gah.

    And.. I didn't get the TB Skin test done :P Very risky of me, but by the time I'd learned about the skin test, my visit to the doctor's was all over! Imagine my panic after that.. But now I'm hopeful that it will no longer be an issue because the TB scare seems to have died down, and it (skin test requirement) seems to be based on the interviewer's discretion. :unsure:

    To confirm..If I am in Macon, GA, I could have my shots and test at Forstyh, GA?

    If that is the closest place to where you live that's where I would go (that's how we picked the place we went to).

    Novie did not have any swelling from the TB test, but here shoulder was sore for several days from the TD shot and she had flu like symptoms for one day after getting the shot.

    Just got back from our AOS interview, the lady who interviewed us said the greencard will be mailed in 10 days. Im really happy :dance::dance::dance:

    That is correct. I had my TD shots in our city health department for free but I had to pay $25 for the TB skin test. Be sure to have these recorded. They will give you a vaccination booklet (I have one from them). Civil Surgeon is the one who will transfer all your vaccination records. I must warn you... since we came from the Philippines where we had BCG shots when we were kids, it is possible that you are positive of TB (I am) because of the anti-bodies. They might insist to have you treated so better bring your X-ray film the day that they will read the TB skin test. That way, you have a proof that you don't have TB and your lungs are healthy, according to the chest X-ray you had in the Philippines. That could save you a lot. :thumbs: Same thing with the Civil Surgeon. Some (or most) of them will insist that you do the entire medical so you have to be firm that you only need the I-693a and that you had the medical at St. Lukes and all the documents were forwarded to DHS when you turned in the yellow envelope at POE. The PE we had during K1 process is valid for 12 mos. so no need to redo the entire physical.

    Jeng -> you can have the TB skin test at a center/local health department, and you will have to bring the results to the Civil Surgeon for transfer of the results/ medical record to be submitted for your AOS.

    You can have the TD shot done by any doctor. Just take the record of such to the DCS.

    Try your local heath department. Some to it for free.

    Ok..that is great...we could have it for less charge with a friend doctor in church...

I do not have one...I admit that my mom accidentally opened the envelope (but i never looked the content so i wont have a guilt feeling)...so I returned the packet and my visa to the USE on the very next day...they reviewed it again and gave everything back to me a week after...thought that was it...but as ive said, it is just the yellow envelope and my passport/visa..this happened in may but i will only be leaving this sept. i just accidentally saw a forum talking about this IDS...

    Well, as far as I can remember, there's a small yellow envelope attached to my big yellow envelope (the one that you're not supposed to open). It contained some photos, which I assumed the ones we included in our I-129F application. I didn't detach it, not even removed those pictures even though that small envelope was unsealed.

    no small envelope 4 me... although, when they gave the re-sealed evelope to me, they returned the pics that my fiance sent with the I-129F but the pics were just handed to me & not inside an envelope.

Is a one-page hand-written and signed medical certification of my private physican, who gave me the 2nd doze of varicella vaccine here in the phils., enough or sufficient to show the civil surgeon for the transcription of my medical records? He indicated therein the date, the dosage and his professional number and details. Do I still need to indicate a photocopy of his professional ID? Does it need to be notarized?

    Please give me advice.
