This doctor above said on the phone that he could do it but eventually I chose this doctor that was closer:
    Dr. Luis Anez
9765 San Jose Boulevard Suite 102
    Jacksonville FL 32257
     904-260-5757
    150 $
3 days wait, after I paid and had a basic examination (not required but they would not fill the form without at least doing that) I had to return the day after and wait there for about 1h while they filled the form.
Even though as first thing I showed them the USCIS guidelines that say that I do not need a new medical and that they only have to fill parts 6 and 9 and let me sign in front of them part 2, they filled also the other parts of the form (probably to avoid to be sued in case of problems) and they added in the remarks section of part 7 and part 9 "PATIENT REFUSED ALL REQUIRED SERVICES FOR EXAM.".
    Hope it's fine..!!

PS. I brought my own I-693 form already filled in the parts that were up to me to complete but they copied them into a new form.

    Why do you need one?

I need it because I never got the vaccination or the chickenpox disease. They did not sell it in the country where I was living (UK).

I just got it today for 150 $ (+ 150 for the second shot) since I don't have a health insurance yet.

I need a physician to certify my vaccinations through the form I-693 without doing again the full examination.

    Thanks

  3. Hello everyone,
I entered the US with a K1 visa and got married to my fiance'. I now need to apply for Adjustment Of Status and need a civil surgeon to certify the form I-693.

I live in Jacksonville and the ones I called want to do the whole medical from scratch even though the one I did before immigrating is valid for one year.

Any suggestions? Any contacts of a physician in North Florida that only charges for transcribing the vaccinations and the required parts of the  form I-693?

    Thanks :)

You are supposed to returned it to the embassy in person and they will correct it ASAP. They accept cases like this as emergency. My friend K1 was also given a different nationality and he took it back and they apologized  and cancelled the old one and printed new one on another page.  Don't make a mistake and travel with it to the sates without the correction because it could be deemed as falsification of documents by you. Remember every details on it must match with what your passport says.

I did not make any mistake, neither on the documents I submitted or on my documents. The embassy did, so they can't accuse me of falsification.

The US embassy is closed for bank holiday on Friday and Monday. I managed to get through with a phone call. They said that even if I go there tomorrow morning as soon as they open, it takes them a couple of days so it would be ready only next mid-week.
The supervisor I talked to claimed that I should not have any problems, laughing he added that the officer will not send me back to the UK just for that.
I insisted on something in writing and they sent me an email (quoted below) that I can print out and show to the officer since the embassy will be closed when I land - meaning that they can't be called by the immigration officer to verify if he wants to.
The email though has no signature.. worst case scenario I guess that I'll have to wait until 3 in the morning until the US embassy in the UK re-opens?

Fingers crossed. And very frustrating that after almost 1 year waiting I have to freak out last minute for their mistake.

    Dear Mr. XXXXX,

We note your Nationality has been entered incorrectly on your K1 visa, this has been entered down as GRBR but should have read ITLY. We understand you are traveling to the United States within the next few days, unfortunately for us to amend this error,  the passport would not be returned in time for your travel.

  5. HELP!! I just noticed that the US embassy put the wrong nationality on my (K1) visa. GRBR (where I live) instead of Italy (my actual nationality). It was their mistakes, all my documents are correct. What should I do??

The US embassy (in London) does not take phone calls, they only reply to communications sent through their website and they take 3-5 days.

I already booked the trip, the wedding and the honeymoon.

What happens if I arrive at the POE (point of entry - NY in my case) with this visa? Will they send me back? Can I ask them to fix it when I'm in the US?

    Thanks!!

  6. UPDATE:
Got my medical examination yesterday. They did not seem to care about documentation from my home country. They did not take my background check (translated) certificate from Italy and were in doubt whether they needed the (translated) "proof of good health certificate" that I managed to get from my last doctor in Italy. I insisted because I knew I had to report 2 hospitalisations on the questionnaire and didn't have further documentation on those with me.
All went smoothly. Last step, interview at the Embassy, is next week. Fingers crossed.
    :)

  7. Hello, how does it work with the affidavit of support for the interview at the US embassy? I can just print the filled I-134 sheets and bring them to the interview together with the proofs (bank balance, employer letter, etc), correct? I don't need to file it mailing it somewhere, right?
(It's for a K1 visa at the US Embassy in London)
    Thanks

  8. Follows the conversation that I just had with an online immigration lawyer:

    GABRIELE:
    Hello,
I am Italian and I have been living in the UK for almost 3 years
Last summer my fiancee filed for a fiance' petitiont to the US immigration, as part for the K1 visa process. We got NOA1 and NOA2.
As expected, I booked an appointment for a medical examination with a physician in London. This is a requirement for the next step that is the interview at the US embassy in London.
The physician sent me a confirmation email with a list of documents that I need to bring on the day of the examination. The confirmation email states that I need to get a Patient Care Summary from my GP and bring it to the medical examination.
This is a new requirementand is not present on the Embassy website in the list of necessary documents to bring to the medical examination since this new requirement was introduced only in April 2017 and the UK embassy website is not up to date.
It is not specified by the physician whether the candidate has to bring to the medical interview the medical records obtained from other countries where he lived or in my case from his/her home country.
So the question is: am I obliged to bring to my medical examination (at Knightsbridge Doctors in London) my medical records from my home country? As said above this is not specified in their requirements. They only ask for the UK patient care summary in their list of required documents.
    Thanks

    IMMIGRATION LAWYER:
If they have not asked for such medical records, then you are not required to provide them. Don't confuse medical records with police clearance letters. You should be fine with just the medical records from the UK.
    GABRIELE:
I am not confusing medical records with criminal records. It is required to bring the police records (ACRO) to the medical interview and I already got those both from the UK and from Italy. They are both expressely required for the interview at the consulate but the list of required documents for the medical examination only quotes the UK one. That's why I believe that's only a generic list usually provided to British citizens and does not account for other cases like mine.
It sounds odd that they don't need the history from your home contry.
Bear with me, I could have been an alcoholic, an addict, have had serious mental problems then moved to the UK for 6 months and applied for a US visa there. My Patient Care Summary would be clean and the US Immigration never know of them. It seems strange.
On the other side it's also true that I would have to provide medical proofs for all the countries I lived in (and could be many) and get records from private doctors too that would not be visible to their checks either.
Is your answer backed by experience with other cases of foreigners living in the UK that went through a US visa process?

    LAWYER:
I understand. I only mentioned criminal records because the U.S. requires criminal records from ALL places where the Beneficiary has lived longer than 6 months. The only reason that I could think of that you are asking if that was the case for medical records was because I believed you might have been assuming that for medical records it is the same as criminal records and it is not. My answer is based on the law. Why would you assume that they want medical records from Italy if they are not specifically asking for them?

    GABRIELE:
Because one of their main goals is to check whether you are eligible to get into the US and without full medical history how can they do that. It's true though that they don't even want the full medical history from the UK, only the summary but it's also true that for the UK they can also double check every answer you gave checking on the NHS system - UK medical system.

    LAWYER:
I understand, but you can hide previous criminal records that could make a person inadmissible. You can't do that in a current medical examination. I'm not a medical doctor, but that's the thinking. So just submit the medical from the UK. Don't overthink this if they aren't asking for more.

    GABRIELE:
Well a person could be an alcoholic, addict or psycopath and not have any criminal records. I'm just a little concern that they'll ask my medical history from Italy and cause even more delays to my visa process..

    LAWYER:
I understand, but that is screened with a CURRENT medical examination. Do not over worry.

    GABRIELE:
Yes, I'm probably (definetively) overthinking but I don't want surprises :)
No, drug tests are not performed by default. They are performed only if there's history of it in the medical records or if the physician gets the impression that the candidate is lying..
(I have nothing to hide, just concerned about delays)
True, that they can spot alcohol abuse from the blood exams and (maybe) psychotic behaviour during the interview. Is that reliable enough though?
I hope you're right and those documents from Italy are not needed. Thanks for your help :)

Here you can find the full list of required documents: https://expatriates.stackexcha…
This is the medical questionnaire I have to fill prior to the medical examination: https://uk.usembassy.gov/wp-co…
As required by that form I have to add an attachment explaining why I have been hospitalised.
    1. car accident, Italy
    2. head trauma with temporary amnesia, Italy
    3. kidney stones, UK
Could they not ask for my Italian medical history given that I have to report those 2 hospitalisations in Italy?

    LAWYER:
They could ask if they feel that there might be something in there that would make you inadmissible to the U.S. Here is a link so that you understand more:

  9. Hello everyone,
I am in the process for a K1 visa and have my medical in London (Knightsbridge Doctors) in 2 weeks.

I am from Italy and have lived in the UK for almost 3 years now. I have all the documents necessary for my medical. I recently got from my GP the patient care summary too (AS THIS IS A NEW REQUIREMENT STARTING FROM 2017) but I was wondering whether I need my medical records from Italy too, it's not specified anywhere!

Follows the list of documents that were specified in the confirmation email sent by Knightsbridge Doctors.
I suspect that this is a generic email for British citizens? But what if a British citizen lived abroad for a few years and then applies for a US visa through his consulate in the UK though?

You are required to bring the following for each applicant having a medical examination:

If you do not have these documents we will not be able to send your medical to the US Embassy and this will cause you a delay.

    1. Original Passport or UK Driving Licence (photo card not paper licence)

We do NOT accept copies of ID

    1. 4 X Identical Passport style photos - UK or US size

(we accept UK or US sized photographs)

    1. ACRO Police certificate (or a copy) dated within the last 12 months for anyone 16 years of age & over (if your police certificate is more than one page long please advise Knightsbridge Doctors ahead of your medical as you may need additional time with the Doctor. If you fail to tell us ahead of time you may be asked to come back to go over this at an additional fee).
    1. Vaccination records - Please provide a vaccination record for the panel to look over on the day to advise if any further vaccines are needed.

Please bring Red Books, if available.

    https://uk.usembassy.gov/wp-content/uploads/sites/16/2015/11/vaccine_chart.pdf

Please visit the above link to see which vaccines are required for each age group.

    1. Completed medical questionnaire which can be found at; https://uk.usembassy.gov/wp-content/uploads/sites/16/2017/05/USA-MEDICAL-QUESTIONNAIRE_IV.pdf
    1. A Letter/Email from the embassy/NVC to confirm your case number and visa category
    1.  Summary of you medical history from GP (NOT full medical records) & reports covering any medical conditions, disabilities, and prior serious illnesses you have been treated for or anything ongoing. This has been a requirement since March 2017 and are aware it may not yet be added to the checklist online or on the US EMBASSY LONDON website.

Ask your GP for a Patient Summary/Care Summary.

    1. If you wear glasses or contact lenses please bring them along to the appointment
    1. If you have any significant/ongoing/previous medical conditions please ensure that you bring a report from your GP/Specialist/Consultant regarding this; including, medication, prognosis, management etc.

    * If your medical notes are very extensive (more than 10 pages) please inform Knightsbridge Doctors before you come for your medical. This required extra time with the doctor to go through your notes. If you do not inform us of this, you will be charged for an appointment slot for the doctor to read these notes.

Please follow this link; https://uk.usembassy.gov/visas/family-immigration/medical-examination/ where you can find all forms required at your medical from the US Embassy website.

    Thanks for your help! :)

  10. Hello everyone,
I am in the process for a K1 visa too. I have my medical in London in 2 weeks. I have a few doubts:

  11. Hello everyone,
I am in the process for a K1 visa, I have my medical in London in 2 weeks. I have a few doubts that are stressing me out.

I recently got from my GP the patient care summary too but I was wondering whether I need my medical records from Italy too, it's not specified anywhere! (It is specified instead for criminal records).

Question 1 on their questionnaire (https://uk.usembassy.gov/wp-content/uploads/sites/16/2017/05/USA-MEDICAL-QUESTIONNAIRE_IV.pdf) asks if you've ever been hospitalised and I had to put YES: I got into a car accident in Italy (been in hospital under observation only), had a head trauma with temporary amnesia in Italy and a kidney infection in the UK (due to episode of kidney stones). I explained all these 3 in an attachment to the questionnaire. Do I need to find and take with me all the translated documentation from Italy? (it would be a problem to get them in time..)

    2. In case I find out only when I'm at the medical that they also need info from my Italian records can they simply access them themselves or I have to provide them (translated) afterwards and therefore causing delays in my visa process?

    3. I put NO on the question that asks "Have you ever had any kidney or liver disease" as it was only a single episode, nothing chronic. Is that correct?

    Thanks!! :)

  13. Hello everyone,
I received my NOA2 and am getting my self ready for my medical. I have a record of my vaccinations and just want to confirm what I will need before Knightsbridge.

    1) I never had the MMR vaccination (Measles Mumps Rubella). I had Measles and Mumps diseases but not Rubella so I need to get vaccinated for the latter.
Is it worth getting tested for Measles and Mumps antibodies (65£ for each test at Knightsbridge) to prove I don't need vaccines for these 2? Single vaccines for these are not sold separately anymore so I would probably need to get the MMR vaccine anyway, correct?

    2) I never got vaccinated for Hepatitis A, Influenza, Meningococcal, Pneumococcal and Rotavirus.
According to the UK embassy (https://uk.usembassy.gov/wp-content/uploads/sites/16/2015/11/vaccine_chart.pdf) you only need them if you're a child. Do I need them since I never got them as a child?
(Vaccines required according to the US Department of State: https://travel.state.gov/content/travel/en/us-visas/immigrate/vaccinations.html)

    - (I never had chicken pox and didn't get a vaccine so I will need to get it done before AOS - it's not sold in the UK)

    3) Is getting vaccinated at Boots cheaper than Knightsbridge? I couldn't find the prices for Boots but other vendors have similar prices. Eg. http://www.nomadtravel.co.uk/pg/42/VaccinationPrices

    Thanks!! :)
