  5. Posting an update: Went to Pag-Ibig yesterday with my Provident claim form, sworn declaration, and a photocopy of my visa. Lady at the counter asked me to photocopy my ID at their copy room, asked about my work history (I worked in one company for 8 years), and asked if I'm leaving the country for good. Minutes later, she gave me the claim form for the check which I can receive in 10 working days.
  6. Thank you. I was able to book my flight and confirm my pet through Korean Air. They advised me to call the quarantine office of Manila (my origin) and Seattle (destination). I also learned from someone that they have a transfer zone in Incheon airport where my cat and I can stay. No further requirements.
  7. Most of you probably already know this but I called the Manila US Embassy a few days ago and they said that a scanned copy of the signed I-134 is fine, if you don't have the original.

Here's what I've gathered from the internet about importing pets to Korea: The pet needs to have a rabies titer test (the Philippines is not rabies-free) which has to be done in an approved clinic in another country Pet must have a microchip that meets the standards Now are these necessary even if it's just a layover? In your experience, did they have to quarantine your pet? Did you have to secure a permit over there?
  10. Called their hotline last night and here's what I gathered. 1. Go to pagibigfund.gov.ph 2. Scroll down to Downloadable Forms. On the left, choose Provident Related. Under Claims, download Application for Provident Benefits (APB) Claim. Download, print and fill this out. 3. Submit the form to any Pag-Ibig branch. Bring two valid government-issued IDs and a photocopy of your passport. Idk how long til they issue the check. I'll be submitting my claim next week to find out. Ok, according to their Provident Benefits Claim charter, it can take 8-20 days. Apart from a photocopy of your passport, they also require a sworn Declaration of Intention to Depart from the Philippines permanently (notarized, no need to submit of already based abroad). This is also in their downloadable forms.

  12. Hi all's! Is the K1 fiancee required to submit their petitioner's most recent federal income tax return form (Form 1040) along with the I-134 to the embassy? Saw this posted in 2017 on a Facebook support group. BUT according to the I-134 instructions from USCIS, Form 1040 will only be required if the petitioner is self-employed. Please help? Scrambling to gather all the paperwork before my interview.
