I can't seem to paste the url here for some reason, but if you go to travel.state.gov and use the Search function (upper right-hand corner of the screen) and search for "Sample DS-160", you get a link to a sample .pdf DS-160.  You can't fill this out and upload it, but it gives you a good idea of the basic form.

The actual DS-160 is an interactive form, so what questions you get may differ somewhat from the sample in the .pdf.  Based on answers you submit, you will get a different subset of questions (for example, if you select that you are applying for a student visa, you will get questions about your school history that you wouldn't get if you were applying for a visitor visa).

If you're applying through USPS, you'll find this useful, it has all the information you need including some of what's listed below: https://www.usps.com/international/passports.htm

You can actually fill out the application online through the DHS website: https://travel.state.gov/content/travel/en/passports/requirements/forms.html (e.g. DS-11 Form Filler) and print it out. They'll give you a bunch of prompts, after filling it out and confirming things you'll be given a download link for the PDF. This likely make it a lot faster for a passport to process since there needn't be someone to transcribe the written info. Also DO NO SIGN the printed form, only sign after the clerk at USPS (or wherever you do the application) tells you to sign it.

If you're applying for USPS, there'll be two payments. One will be to the U.S. Department of State which needs to be a a check, money order or stuff like that, and nothing else (e.g. no cash, debit/credit etc...). As for the USPS fee you can pay that in any way you wish, that's typically $35 dollars excluding if you need your passport photo taken (another $15, but rates may vary). The USPS website above will give you all the info you need.

As for updating your SSN record I believe a passport will suffice (maybe your old SSN? I'm unsure). But basically you just have to show up to a local office with your proof of citizenship (e.g. passport), ideally at least 10 days after your oath since it takes time to update.

Thank you so much for your helpful answer. How about register vote? can i do it now through online?

  4. Hello, I have few questions here :

i am about to apply my passport. Does the DS-11 form has to be print out and i write with pen or i can type everything and print it out because i wasn't able to fill the form online on PDF like i did with my N-400.

And what kind of type payment do they accept? Cash / debit only or can use credit card also?

And what do i need to update my SSN at the office? Do i need to fill out any form ?Or when i get to the office, what the reason i should tell the employee? To update my citizenship status?

    Thank you

  5. Today i had my interview and i passed. It was very fast about 15 minutes. The IO didn't asks my husband's birth certificate ( i sent it last year with the paperworks ). She just asked about the recent tax 2017 because i haven't had it when i sent the files. She took my pic and fingerprint ( left & right index ) and swore me.

Asked me to read and write the same sentence. And asked me 6 questions out of 10.

    1. What's the rule of law

    2. How old do citizens have to be vote for President

    3. What major event happened on Sept 11, 2001 in the United States?

    4. What ocean is on the West Coast of the United States?

    5. Why does the flag have 13 stripes?

    6. What's the name of the National Anthem?

I got the form N652 and now one step to go is waiting for the Oath Ceremony. Good luck to all of you.

Please could somebody update my status :

    Interview letter 8/10/18

    Thank you

Oh well. I was interviewing with the very nice African American female officer. She escorted me to her office (while I observed other officers were just letting the applications follow them without saying a word).

She went over with my application first (after placed me under oath, and asked me fore identity forms + interview letter)

    (1) Asked my general information such as: name, birthday, current address...

    (2) Asked where I worked, and lived since I came to US.

    (3) Asked if I was out of US. (I said no during the past 5 years because I love US that much, she was so surprised and she opened my passport in order to verify and even asked: Not even to Canada or Mexico ? - I smiled and said No. )

    (4) She went to those Yes/No questions.

    (5) She asked me to review the N400 form with a lot of red pen revision.

Then she went to the Civic test, asked me these 6 questions:

    (1) Who wrote the Declaration of Independence ?

    (2) Name 1 state that borders Mexico ?

    (3) Name 3 original states ?

    (4) What are two major political parties in US ?

    (5) Who is Commander in chief of Military ?

    (6) What is the party of our President right now ?

For the writing test, she asked me to read out loud: Who elects the Congress ?

She gave me an answer and asked me to write on the paper: The People elects the Congress.

    She repeated the answer twice.

She asked me verify the information includes my name, my address.. which will be in the Naturalization Certificate, and sign it.

She also asked me to sign form N400.

After that, she congratulated me and recommended me for an approval.

I have to move next week so I asked her if I could have any options. She went out and asked her supervisor and gave me the option of

    (1) Process the application here and take Oath at my new local USCIS.

    (2) Come back for the Oath Day.

I said I would not want to be take Oath at my new address since it would be a huge delay so I politely asked her to process my Oath in SF.

    Hope this helps. LOL

I have done my interview on 9/18, and still waiting for Oath. Super easy interview. What is your field office?

yes i just clicked your hidden contents :) and saw you already had the interview. Which 6 of questions did they asked you? And about the writting test, did they asked you to write all the sentences they said? Did they only says it once or repeat it? I will sending the paperworks maybe after Christmas due the severe weather in my area ( snowing too much ), so it will be better maybe around January so i can get my biometric after winter ended :P

my Field office will be in Reno NV

In the N400 form, you might fill as much as you could. An adjudication officer will probably verify all the information with you.

For example, IO might asks you about the name you have been used and you might ask her/his about this. He/she will correct it with the red pen.

I suggest you should have to fill your maiden name in that part.

Ok better be safe than sorry. Thanks Rlam

     8. correct 3 times

8F. If there is no more room ( just need dates ie. married 01.21.2000 divorced 01.21.2002) a separate piece of paper is fine.

    7A. correct. No.

7B. your answer was no in 7A. so just leave blank or N/A.

    Thanks Enigma :)

One more question: Part 2 : #3 : Other Names you have used since birth : Last name ( Since i changed to my husband's last name, should i still have to fill it my Maiden Name )?

  10. Hello, i have some questions regarding N-400 form.

Part 10 about Your marital History :

    # 8 : How many times has your current spouse ( my husband ) been married ( including annulled marriages ... ) >>>> he had been divorced 2x so my answer should be 3 ( including married with me )

#8 F : My Current spouse's Date of Marriage with Prior Spouse & Marriage Ended with Prior Spouse(s) >>> Because this had been 2x, so i need to explain it on the separate letter?

Part 12 : 7A : have you ever not filed a Federal, State, or local tax return since you became a lawful permanent resident? >>> No ( i filed with my husband )

Part 12 : 7B : If you answered Yes, did you consider yourself to be a non U.S Resident? >>> Should i leave it blank or answer No?

Thanks in advance for your help.
