  1. Hoping someone might be able to help me. 

    I need to get my medical records sent over from UK to my GYN. I am having a hard time finding out how to do that. 

    I am looking for the records back in the late 1990's before I came to USA to live.

    How can a person apply for access to their health records if they are not living in the UK?

    In cases where a patient moves abroad, their GP health records are sent to the local Primary Care Trust and their hospital records are stored in the hospital or sent to a local archive. They are retained for the minimum period. Hospital records are kept for a minimum of 8 years following the end of treatment and GP records for 10 years, though please note that certain types of records are kept for longer. At the end of the minimum retention period the Records Manager at the Trust/hospital/archive will decide whether to retain further or destroy the records. Under the UK Data Protection Act 1998, a patient has the right to apply for access to copies of their health records. The request should be made in writing to the record holder(s), and a fee will be payable, with the maximum that can be charged set at £50.

    Question

    Can a person take their health records with them for their new doctor, if they are living outside the UK?

    Answer

    No. however patients can make a subject access request for copies of their records, and then take the copies with them. The GP may be prepared to provide the patient with a summary of the patient’s treatment to take to their new GP.

    I see it says make a request in writing to the record holder. But does that mean at my local hospital? 

    I'm thinking that my records may have been destroyed by now. ugh.

    Passports are renewed only in the UK. The old way through Washington DC is no longer available. You will have to mail your current passport and two passport photos off to the UK. Extra fees (around £20) charged are to courier things back to you. This page is where you start when you are out of the UK and need to renew. https://www.gov.uk/overseas-passports

    When they say read guidance notes and provide a link, then do it. Same for a link to supporting documents.

  3. Hi all its been a while. I'm a permanent resident and my passport expires in September of this year.

    We are going on holiday to UK in June so I need to renew my passport.

    The last time I was able to get it renewed at the UK embassy in Washington DC.

    Does anyone know how to renew passport now? It seems that you can't do that now.

    Does anyone know the processing times if the passport is sent to UK?

    Thanks just after any info am finding it difficult to find the info. And get redirected to websites that want astronomical fees.

  4. Ok so first off I'm hoping this was the right place to ask this! Apologies if it isn't please direct me to the correct forum.

    I haven't been at the forums in a long long time lol

    Anyway, I am helping an elderly lady renew her I-90 and I noticed that there is a form I-912 Request for Fee Waiver.

    Now, Section 3. Basis for Your Request Line 7. b. My household income is at or below the 150% of the Federal Poverty Guidelines...

    IS SOCIAL SECURITY INCLUDED with her household income? She works part time.(I read over the instruction sheet but don't understand, perhaps I missed something. I know it is not included in the Means-Tested benefit [Line 7. a.])

    Before anyone asks why isn't she a citizen please don't ask...she has been denied multiple times sadly because her English language is not that good and shes been here since 1959. She was so upset the last time she was denied she doesn't want to apply again.

    I want to help her any way I can so if there is info out there please could you include that in your kind replies :)

  5. Regarding the confusing new N-400

    Should those who answered "NO" to Q 38 "Are you currently a member of the armed forces?" answer Q 39 and 40 (both of which begin with "if you are currently a member of the US Armed Forces"), with "NO" or leave them blank?

    - answering these 2 questions assumes you are in the armed forces whther you say YES or NO

    - not answering them violates the guideline to answer all questions

    Same for the many "if yes" questions. Should the A, B follow-up questions be left blank?

    Q 17 "Were you ever part of any group..." NO

    A. If yes....

    B. If yes,...

    I am having this problem too!!

    What do I do? Leave it? would seem real stupid to say No to say something that contradicts the first answer you gave!

    5/31/2012: sent the N-400 package

    6/02/2014: package was delivered

    6/02/2014: 90 day point (eligibility based on 5 years)

    -------------: Receipt Date /Priority Date

    -------------: Check Cashed

    -------------: Notice Date

    -------------: Biometrics letter mailed

    -------------: Walk-In Biometrics

    -------------: Biometrics appointment

    -------------: In line for interview

    -------------: Interview letter mailed

    -------------: Interview appointment

    -------------: Oath-taking (God-willing!)

    I am going to send in my form soon. I'm filing N-400 based on 5 year rule.

    Are there any other supporting documents I need to send with the form?

    Also am a bit confused when it comes to filling in the form do I need to put NA on everything that I left blank?

    example...the additional room to add other addresses or children

    I have the new form with the bar code and I can't add the / to N/A it won't let me, so do I have to write that in by hand or is leaving it blank ok?

    It just seems a bit tedious to do this. I would rather just leave it blank.

  7. Recycling paper is not good for Wisconsin loggers with the further degradation of the environment by using all that bleach that ends up in rivers and streams. Don't believe everything you hear. Paper is fully biodegradable, real crime is in plastic bottles and bags.

    Am laughing hard here!

    I had the same question double sided (to save paper) or single....

    I guess single wins it!

  8. Yeah, I believe you could go either way whichever you prefer. You qualify for citizenship under both cases, so it is your pick. The fact that you originally got your PR status through marriage to a US citizen in this case no longer weights any importance over it anymore since you "abused" your stay! ;)

  9. Thanks! I got my GC through my spouse but I've been here so long now. I just thought it would be easier to go through the 5 year route.

    I am long overdue...life just got in the way ha!


    If you got your GC thru marriage to a US citizen, i would check the PERMANENT RESIDENT AND SPOUSE OF A US CITIZEN

    Why would I check both because on the form it specifically says only to check one.

  10. Ok, So I've gone and filled in the N-400 form.

    I've had my Permanent Resident card for 8 years so...

    NOT SURE WHETHER I SHOULD HAVE CHECKED OFF on Part 1. Have Been a Permanent Resident for at least 5 years OR Permanent resident & spouse of US citizen.

    I checked the first box since it seemed like a lot of stuff to add to the packet filing as spouse of US citizen.

    IS THIS THE RIGHT THING TO DO?

  11. I just have a few questions

    Since I have been married for 7 years and am just now applying for my citizenship - what box do I fill out on the form?

    Been a resident for 5 years or married to a USC for 3 years?

    I am sure I will be questioned as to why I left it so late. The reason is time! My family has been ill overseas and my life has just been so busy here looking after my children. Will this be used against me?

    I hope not. I just want to make sure I am checking the correct box. I would just check married to a USC for 3 years because I have....well for 7 years!

    Advice please!

  12. I just got a call from my mum telling me she received some mail from the DVLA saying that I need to renew my photo license as the 10 years is almost up. I tried to find information for British Citizens living abroad renewing their license and didn't really find any info.

    So, I got married in USA and went through all the K1 ####### and finally have green card. I not only changed my name but obviously changed my address. Do I need a UK address to be able to renew my British License? I still live in USA but I hope to live in England in a few years, so I want to keep my license! My mum is going to forward me the renewal forms. I know I can apply online for address change as long as your license is not due for the 20 year photo expiration - well I know I can't do that so I really have to renew a whole lot! Photo, name change and address.

    I just want to know if its ok to put my mum and dad's address on my license?

    Has anyone else had to do this? What did you do?

    Any help would be appreciated!

  13. I wonder if anyone can direct me to another web site forum that discusses how I need to file for my taxes.

    I am self employed - I clean 2 homes and work 5 hours a week for a Dr. as his secretary which I have been doing since Dec 08. Cleaning I haven't filed taxes as I haven't been earning over $400 per year.

    I don't know if I need to register my so called 'business' I don't know if I have to file a 1099 or 1040. Then which 1040 do I file, does the fact I am a green card holder mean I have to file a different form?

    If I can be directed to the right place please help!

    I need an answer in laymans terms!
