That's an older form expired in 2011. And that example contradicts with what others have written here.
Some parts of the form explicitly say to write "None" which I understand. But that example has fields where they put "None" even though the form did not say to put in "None".
My understanding is that with all the newer forms (129f, 325a), unless it explicitly says to write "None", we should just leave it blank?
Is this correct?
