Here's what we did with the cover sheets.
On the very top of the package we placed the bar-coded CHECKLIST sheet that came in the mail. With all the checkboxes checked. This is your main cover sheet.
Then comes the AOS bar-coded sheet (yes, it also has the words cover sheet on it ), that you print out from the site after paying the fee. Then our own cover letter, detailing the actual AOS documents in the folder. Then all the AOS documents in the same order that they are listed the checklist. THEN IV bar-coded sheet from the site that you print after the IV fee. Then, our own cover letter for the IV docs. Then IV documents (also in the checklist order).
I am not sure if you can use AOS sheet twice. It DID have different printed text on it, and my guess is that each one verifies its own fees have been paid.
To make sure everything is in order, and to help the reviewer, we went as far as placing little colored tabs on the sides, like AOS, Income docs, Supporting docs, etc (again, using the terms from the checklist).
Got approved without RFEs!
