    -90 days +3 years from the "Resident Since" date on your green card, if filing as the spouse of a US citizen that you've been married to for three years.
     
    -90 days +5 years from the "Resident Since" date on your green card in almost all other cases.
     
    For green cards obtained through VAWA (or RoC based on extreme cruelty/battery) it's also three years.

    What's the issue then? Your passport, ticket, and GC are all the same name right?

    Yes, I-485 and I-765 are processed separately and thus, have a different set of required documentations. You need to submit duplicates of similar documents, and yes, 2 additional pictures too. (http://www.uscis.gov/sites/default/files/files/form/i-765instr.pdf , Page 6-7)
    Btw, might as well add in the I-131 (AP) with your application, since it's free when applying with AOS. The requirements are somewhat similar to I-765 too.
