Hey CanGal!
Did your doctor put the I-693A in a sealed envelope for you?
Thanks,
Tom
Hey Karen. You also are going through Memphis are you not? I sent you a message about a week ago, I hope you recieved it. I too am going through memphis. We have our interview on June 23. As for the medical, i just printed the I-693A form off here and took it with me to the doctor (though I'm sure he has the form himself). I believe you do not have to have a medical retaken regarding your situation. All that should take place is having is vaccinations put on the I-693A form from the civil surgeon.oops sorry, your interview in july, not june but good luck on your interview that is ...

Hi OyenJames,
Congrats on your biometrics!
I'm curious though, can you travel outside the USA with your current foreign passport, or do you have to change your foreign passport to your married name?
Thanks,
-Tom
Just wanted to share our biometrics experience at ASC Birmingham today. We were scheduled for 12 PM but since we left Dothan very early, we got to Birmingham at 10:30 AM. I went inside at 11 AM. Filled the form and got a number. Was called at a little after 12 and the lady was really nice. I was done in no time.
Oh btw, I'm using my married name for my AOS but since my passport is still in my maiden name and I don't have any other ID in the married name, I just bring my original marriage certificate (& copy), which they only looked at.
Hopefully, we'll get that interview letter ASAP.

Does anyone know if the I-693A has to be placed in a sealed envelope by the civil surgeon?
On the form, there is a space for the doctor's signature, but it doesn't ask for the doctor's address, license number, or anything else. My civil surgeon just checked the boxes and handed me the form (unsealed).
Thanks,
-Tom
Hi all,
My fiancee is here on a K1 Visa and I'm getting ready to file her AOS. I just returned from the civil surgeon's office with the I-693A, and here is what the doctor wrote:
DT/DTP - Not age appropriate
Td - Not applicable
Polio - Not age appropriate
Measles (or MR or MMR) - Not applicable
Mumps (or MMR) - Not applicable
Rubella (or MR or MMR) - Not applicable
Hib - Not age appropriate
Hepatitis B - Not applicable
Varicella - Not applicable
Pneumococcal - Not age appropriate
Influenza - Not fall/winter (flu) season
Bottom line was that the civil surgeon said that the "Not applicable" vaccinations weren't required, but then he checked the box at the bottom of the I-693A that says "Applicant does not meet immunization requirements."
Does anyone know which of these vaccinations are required by the USCIS or not?
This is really confusing. My fiancee received her medical and vaccinations in her country as a condition to receive the K1 visa. Then, immigration took her package and all her documents at the POE, so we have no papers that prove her vaccinations or medical exam. What a messed up system!
Thanks,
-Tom
Tom
vaccinations are still required. According to USCIS
" K and V nonimmigrant (temporary) visa applicants. Individuals applying for a K nonimmigrant visa as the fiancee or the spouse of a U.S. citizen or for a V nonimmigrant visa as the spouse or child of a lawful permanent resident (LPR) (including those applying for V status in the United States) must undergo a medical exam, but are not required to comply with the vaccination requirements at that time. They will be required to comply with the vaccination requirements when they eventually apply for adjustment of status. "
You may want to check the following link Medical/Vaccination
Is there a way for her to get a copy of the vaccinations she has received, not necessarily the one filled out by the surgeon back home? She needs to show that she has had the vaccines (even if her old doctor faxes over a sheet of her vaccination history so she can show it to the CS).

Hi all,
Here's a question on advanced parole and foreign passports after the AOS.
If my fiancee changes her last name to my last name, will she still be able to reenter the USA with her foreign passport that has her maiden name (as opposed to her married name)?
Or will she have to get her passport changed in her own country to reenter the USA on advanced parole?
Wow!!
Thanks,
-Tom

The G-325A form has 4 color-coded pages. Is it okay to use the .pdf download even though the pages are not color-coded? (I was afraid to try it with the I-129F).
Thanks in advance!

THANK YOU roi-aggie!!
You're awesome!

The Alien Registration number is on your I-797's from the USCIS, and should have also been written on the back of your I-94 by the Immigration Office at POE.It is in the "Beneficiary" block of your NOAs.

Well in the example given at http://www.visajourney.com/examples/INS-Form-G-325A-aos.pdf it appears that the "File Number" at the top of the G-325A is the same as the "Alien Registration Number" at the bottom of the G-325A. Does anyone know where to find the Alien Registration Number?
Thanks in advance!!
-Tom
Does anyone know where to find your file number for the space on the first line of the G-325A?I noticed that there is a number given in the form example at http://www.visajourney.com/examples/INS-Form-G-325A-aos.pdf
Thanks!!

My wife came here on a K1 visa which required a medical exam and vaccinations. The papers for her exam and vaccinations were in the sealed package given to her by the consulate, and the package was taken from her by immigration at JFK. Does anyone know if it is necessary to get the vaccinations again for the AOS since we don't have the I-693 that was taken at JFK?
Thanks!
From VJ:
"If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application."

Hey, thanks RCH99!
Does anyone know if the G-325A, Biographic Information, is required for both husband and wife in the AOS application, and if one can use the same G-325A that was submitted with the K1 application, or does one have to complete an entirely new G-325A?
THanks!!
for AOS it's just needed for the foreign spouse. I would make a new one.

Advance Parole and Name Change
