Philippine Passport Name Change... I badly need help about this.

Hi everyone, I need to change my name in my Philippine Passport because we're going to visit the Philippines this coming March. I haven't reported my marriage yet (got married 8 moths ago) but me and my husband is planning to do it as soon as we receive the authenticated marriage certificate that we requested through mail and we're just an hour away from Chicago.

My big mishap is, the name in my GC and EAD appeared as Cherry Lyn Mojica Burle instead of Cherry Lyn Canlong Burle and my maiden name is Cherry Lyn Mojica Canlong. I've patterned my name to the US way.

I've read about paying $25 for the "one and the same" document for an amended passport. I really want my Philippine passport to reflect the name I have in my GC. Is the "one and the same" documentation going be the only option I have?

Thanks in advance.

-Cherry

I can't answer your question directly but I would suggest you give the Philippine consulate in Chicago a call. I am sure they can help out. Call them at 312.332.6458

The main Philippine consulate in Washington D.C. phone is 202.467.9300.

If your going to renew your passport you don't need to amend, you can apply for name change at the same time report of marriage, check the website of the Philippine Consulate closest to your place...

In your philippines passport you have to follow the philippines rule. The"one and the same" is your main option. This issue had been asked before in VJ, try to search for it.

If example your name in your passport and GC doesn't match, you have to apply(in the consulate) for a document saying "one as the same" justifying that the name in your passport and GC belongs to you (one person).
