UK Applicants - inportant info for you

Please note that if you are a UK applicant you need to ask the Doctor at whichever Nuffield Hospital you go to - TO GIVE YOU THE VACCINATION SUPPLEMENT SHEET AT YOUR MEDICAL. - it is known as the I-693.

When you get to the USA, you have to send this in with your AOS application, if you don't 90% of the time you get an RFE.

It seems that the Nuffield Doctors have not been advised to give you this, even though at the bottom of this sheet it says in small print 'Give this form to the patient'. Applicants from other countries seem to get theirs at their medical!

If you take a look in the AOS Forum you will see this coming up as a problem again and again for UK applicants who get RFE's. They then have to go and find a Civil Surgeon in their area and pay more to get the vaccination information transferred onto an I-693 form.

If I had known this before I left the UK I would have made sure I had the vaccination supplement and form from the Nuffield. Now I am trying to get it by calling the Nuffield from the USA!

Sarah, The Nuffield are very good when it comes to things like that, someone on this forum that i know personaly called them to get copies of the medical for school purposes for the kids, the faxed them no problems!!........FIRED though :lol:

Just a quick correction: I-693A is the Vaccination Supplement form and it is a USCIS form, NOT a Department of State form (since that's who you're dealing with when you're getting your medical before your interview). The DS-3025 is the Department of State's vaccination worksheet.

You SHOULD be given that form, and if Knightsbridge (the new contractor--Nuffield lost the contract) does not give it to you, don't leave your medical without it!

Suzie, does this also apply if you will have your LPR visa as we will?? or is it better no matter what type of visa to just go ahead and make sure you have it, sounds about right that!!

Sorry I do not know about this type of visa, but yes if you are coming from the UK and will have to do AOS you will need to have this to send with it.

I had my medical a week ago, and didnt recieve this form! Are you sure I need it, none of my documents said I neded it from them and they didnt say anything about it. :wacko:

You should receive the form pax spoke of above.

DS-3025.

You can request your records from Nuffield but I am not certain that even if the I-693 is in there, that it will include the vaccine supplement (I-693A) and that is the form causing the RFE's.

ok well now im really confused. I had my medical at Knightsbridge doctors in London, not Nuffield.

I needed to have 2 vaccinations done at my doctor back home, so they were going to wait till I did that and faxed them the letter from my doctor saying id had them till they sent my results to the embassy. I have had the vaccinations and the medical results have now been sent to the embassy.

Upon leaving the medical exam in london, they gave me no forms whatsoever except for a sheet saying which vaccinations I needed to get and a letter to my doctor explainign why i needed them.

From reading all the stories f peopels medicals and embassy interviews, ive seen nothing of this form and no mention of people needing to take it back with them from the medical.....I should probably make a topic about this as it seems pretty important.

Sorry I confused you with Nuffield. Knightsbridge is who you need to contact.

There is a similar thread going right now in the Embassy forum.

Look in the VJ guides for AOS requirements and packet preparation. You will find some more information about this there.

There was a rather lengthy pinned thread about this very topic in the AOS forum on the old board but it seems to have disappeared.

Ok I have just looked in the guides and the information about the supplement seems to be gone. I am certain that it was added this fall.

PM'ing the powers that be to see if this is a board glitch.....

Let's clear a few things up...

1.) When you have your medical, no matter where you have it, you should not leave without a copy of DS-3025. This is the Department of State's vaccination WORKSHEET. You should take with you to your interview proof of all vaccinations you have received in the past, or results of titre tests you've had in lieu of vaccinations (e.g. if you've already had chicken pox or measles); you should leave with an unsealed copy of the DS-3025 for yourself to keep. If they don't give you one, ASK!

2.) When you apply to adjust status in the States, assuming you as a K-1 holder apply within one year of the original medical, YOU DO NOT NEED AN I-693. You need an I-693A. This is USCIS'S VACCINATION SUPPLEMENT. Here's where it gets tricky! Some people have submitted a DS-3025 with their AOS package and have NOT received RFE's. Some HAVE. If you want to be absolutely safe in terms of your medical documentation as a K-1 adjusting status, you should take your DS-3025 to a USCIS-certified civil surgeon and ask them to transfer the information from your DS-3025 to an I-693A. They should do this for a nominal charge. If they try to tell you you need a full medical examination, DO NOT LISTEN. You ONLY need the vaccination supplement. Somewhere out there is floating an official USCIS memo on the subject that's only a month or two old; I'll try to dig it up.

3.) The civil surgeon should give you an I-693A in a sealed envelope documenting the fact that your vaccinations are complete (either they were completed when you had your medical or you has them completed stateside). You should submit this SEALED envelope with your AOS package.

ok, so how are people supposed to know they need to take that form home with them? if its that important then something seriously wrong is happening at the medical examiners. My results have already been sent to the embassy so im assuming its too late for me now? :blink:

can you not just call/email Knightsbridge and ask them to send you another copy to your home addy?

I'm thinking this would be the logical first step to try
