Received Section 221g and need to re-schedule. Need help

Hello, my mom had her interview yesterday and even though I thought I did everything right, apparently it wasn't scheduled properly and she got given a notice of file is incomplete. I've attached screenshot of it below

 

They said I selected the wrong visa class. This is for:

-I am a US Citizen

-Applied for I-130, got transferred to NVC, did the DS-260 and all at ceac site already.

-Bombay embassy so scheduled appointment by going to: https://www.ustraveldocs.com/in/en/

-Selected 'Immigrant Visa'

-Above Step 5 has me go to: https://www.usvisascheduling.com/

-Registered account there and maybe thats where something went wrong?

 

Anyways I now have to schedule two appointments again - one for biometric and one for consulate interview.

  1. Has this happened to anyone before?
  2. Do I have to create a case in the scheduling site for them to delete my applicant and do it again?
  3. I want to make sure I don't make a mistake again so is there a way to do brand new applicant? I tried by the scheduling site said it detected I already have profile

 

Any help appreciated
