Hello, I am trying to apply for a naturalization certificate replacement. I am carefully reading the application and checking each area for the reason and mine happens to be more than one.

First of all, it's lost and I have no copy of the certificate, I changed my name w/court order AND there was an error I've been trying to correct since before but couldnt because prior docs had the same error.

Now for the "lost , stolen, destroyed" reason, it's asking for a sworn statement. Do I have to attach a separate letter or can I use the lines on the application because I don't know where it is and to be honest, I am traumatized by having to go more into it.I have the court order and the corrected document for the other two reasons for applying for the replacement. 

More importantly, for those who may have mailed in the forms n-565 with i-912, did you have to physically go in for an appointment ? I just want to know if I will eventually receive the corrected certificate with new name in the mail or are they going to give me more problems?
