DS 260 Entry Disappears after being saved

CR1/NVC STAGE
In filling out the DS260 question "Have you travelled to any contries/regions within the last 5 years? We answered YES & then entered the 2 countries travelled to in the field. When we proceed & look in the Review section it appears correctly. But when we login again later on the BOTH YES & NO fields appear "unchecked" & the entries are gone ??? There seems to be a bug in this field.
Anyone experience missing data entry while filling up the DS 260 ?
If it persists is there a way to attach a statement or document to show this information in case it does not show up when NVC recerives on their end it & sees both YES & No as unchecked?? Causing them to ask for the missing entry & delaying the process.
