What does the DCII query report (DCII check performed by USCIS) include?

I would like to know what the DCII report shows when the query is performed on the DMDC 6.7 application by the USCIS contractors.

I know it would have a persons basic info,

Apart from that, does this check also show a persons: 

1.       Reason for Separation,

2.       Character of Service,

3.       Changes in Paygrade,

4.       Marital/Dependent Status,

5.       Occupation? 

Or only any derogatory info on his record?

I also read somewhere that we are allowed to request our own copy of DCII query report under the Freedom of Information Act, whom should I send the request to?

 

I read online on a general public website that the request must be sent to:
