CRBA....5 years physical presence?

Hello,

 

I'm trying to apply U.S. citizenship for my newborn daughter thru CRBA. I have couple questions regard to the process.

 

Question 1: 

Can all of the 5 years physical presence requirement be applied after the age of 14?

 

Question 2: 

On the Form DS-2209, item 24 requires to list the "Precise periods of physical presence in the US". Since I no longer have my old passport, do I need to provide some sort of evidences for all those dates because there was 2 short trips I took aboard that I can't remember exact date? 

 

Question 3:

I have comprised of 6 years of 1040 Income Tax with the W-2 and a Social Security Statement which was strongly recommended by the consular officers. Are these evidences considered sufficient proof of 5 years physical presence in the U.S.?

 

Question 4:

Is it better to obtain a DNA test and submit the result along with the application to improve the chance of getting aprroved?
