Need help with form I-90 Part 3 questions.

Hello, I am a Canadian who married an American citizen in 2006. I filed all the paperwork and was granted a Permanent Resident Card (Green Card). My card expires in June this year and I am applying to renew it with form I-90. Part 3 asks 2 questions that have me confused.

 

1. Location where you applied for immigrant visa or adjustment of status.

2. Location where you immigrant visa was issued or USCIS office where you were granted adjustment of status.

 

So I have a really bad memory and I cannot find my original paperwork. We have moved, I have 3 kids, things got lost. I have no idea where I mailed the original forms. I remember that we had an interview in Indianapolis, IN. I also remember that the first green card was only good for 2 years and then the next one I got was good for 10 years. Through some internet searches I found that you can see the USCIS office on the back on your green card. If I tilt my card in the light I can see in the microfilm Card # WAC123456789 (Not the real numbers.) The WAC suggests California Service Center. Would this be correct? I don't remember sending anything to California. I was thinking I would enter Indianapolis, IN for #2.
