 Hello all,

 

 My wife is trying to renew her Philippine passport and in doing so we discovered that she was supposed to file something called a Report of Marriage (ROM) for the Philippine government. Apparently this processed document is a requirement for a new passport with a name change due to marriage. Since it has been several years since our marriage, one of the ROM requirements is a form called Affidavit of Delayed Registration of Marriage (link below). This is easily the least professional form we have seen throughout her entire immigration process (made amazingly easier with the help from you wonderful people here by the way), someone just whipped it up on Microsoft Word and said "eh, good enough". To make matters worse, this form is different than other Philippine embassy forms of the same type. The Los Angeles embassy form is completely different than this one and all searches for a sample form have turned up very different and not related results.

 

 Could anyone give us a hand navigating this form? We have tried calling the SF embassy every day, dozens of times a day for weeks and cannot get through. We have sent requests for assistance on their online help request from the official website with no response yet (it is supposed to take up to 4 days, it has been 3 weeks)

Here are our questions, line by line from this form;

 

1. her married name or maiden name?

2. full street address?

    [a] (this we get, it's me)

3. , [c] does the date nomenclature matter?

    [d] (we know this)

4. [e] We plan to simply state that the delay is due to not knowing that a ROM was needed, we hope this is acceptable. (At no point did the Philippine government inform my wife that it was mandatory, and of course the US gov didn't either)

5. (everything starting with "IN WITNESS WHEROF" and beyond) This last one is the really tough one for us. We don't even know what it means or what they are asking for. This form is to be notarized, so is this some kind of message for the notary? Is it for my wife to sign? What does it all mean?

Hope this helps.

First of all, check the San Francisco embassy if the county of your marriage certificate falls under their jurisdiction. In my case, I had my papers sent back to me because I'm a resident of San Francisco Bay Area, but got married in Las Vegas, which is under LA's jurisdiction. (Don't want you making the same mistake that I did. If you already know this, proceed below).

 

1. her married name or maiden name? - I hyphenated

 Yes, we were married in SF so that embassy is definitely the one for us.

 

 So the last bit is something my wife is to sign, presumably in front of the notary.
