Questions regarding submission of Documents to NVC

Hello can someone pls answer me following questions:

1-Do we have to submit any other document to NVC with "document cover sheet" Or we have to provide only those documents that are listed in document cover sheet? Like copy of passport or pictures or any other etc pls guide me.

2-We have two same document cover sheets sent by NVC, one in the name of my wife and one in my name but both are same. Do we have to attach only one document cover sheet?

3-We have a joint sponsor in our case, do we have to attach separate document cover sheet for joint sponsor's affidavit of support or we have to attach only one document cover sheet for all (main sponsor, joint sponsor and for supporting documents), we have to use only one document cover sheet.

4-Do we have to attach payment receipts for IV fee payment and AOS fee payment?

3-Do we have to write case number on top right corner of every document including affidavit of support.

In above questions Document Cover Sheet means check list on which we have to mark check boxes
