Hi,

I am new here. I am now collecting all the document for filing AOS.

I entered the US with my K-1 visa, and I had my medical exam in Sept 2006 in my homeland, the doctor just gave me a sealed envelope and it was given to the immigration officer during my entry to the US. I have nothing else from the doctor except a chest XRay in a sealed envelope.

I remember that 'vaccination exempted' was written on the instruction for my medical exam, when the consulate sent it to me and requested me to do that exam last year, what does that mean?

Could anyone please tell me if I need that vaccination supplement I-693A.

And how can I obtain a I-693A if I really need one? Please advice, thanks a lot!

Hi and welcome to VJ.

As a K-1 you are required to submit the I-693 with your AOS application. You do that by taking the vaccination worksheet to a civil surgeon here in the US and they transfer the info for you for a nominal fee. That being said, many people just submit a copy of the vaccination worksheet they got when they did the medical but in so doing, you could risk a RFE.

However, if what you are saying is that you did not receive a worksheet at all, then you are in a different boat. Are you sure you did not receive a piece of paper along with your x-ray? From everything I have read here, it is a must, not an option. Perhaps you could contact the doctor that did your medical and request a copy.

Feb 25 - got emails saying that the cases had been transferred to another office for processing

Feb 26 - got emails saying that the cases have been transferred to my local office for processing

Feb 28 - got emails saying that the cases have been transferred and are being processed

Mar 17 - got email, attached to one case number only, saying that my A number was changed relating to the I-130 filing

Mar 18 - got emails saying that the petitions are approved smile.png

My husband didn't get one either. All he had was the 'prescription' telling him which shots to get (and the mini x-ray). We had to call the doctor, and his brother had to pick it up, fax it to us, and mail the original (which we're still waiting for). We didn't know about Visa Journey back when he had his physical, so he didn't know to ask for the vaccination information.

I finally went to a civil surgeon in Virginia to fill out the vaccination supplement and had 2 shots last week...he said that mine were received too long ago...:s

What kind were recieved too long ago? That sounds kinda strange.

I had to have a tetanus-diphtheria (Td) shot as well .. But i did it in Canada, before my K1 medical .. the nice nurse told me to make sure i had one before coming to my appointment ..
