Which document cover sheet? They gave me two different versions!

When I paid the AOS fee, I got a document cover sheet that has the list of documents, where i can check "Enclosed", "Unavailable", or "Not Applicable"

But then when I paid the IV fee, I got a document cover sheet that states at the top in quite large letters, "DOCUMENT COVER SHEET" The text is much bigger in this cover sheet. However, there is no place on it to list enclosed documents.

Both cover sheets have bar codes, visa type and case number. That info appears to be identical.

Which do I use???

I've attached the actual cover page they sent for the IV. Different than the AOS one!
