Yes i did change my family name for divorce.

Do you guys have the checklist if i'm applying for 5 years rule divorce/

I don't need to send tax returns?

So i will put my birth name then?

Yep.

Your current legal name will by your birth name.

Where it asks for the name on your green card, write it exactly as it is shown on your green card.

Where it asks for former names, you'll list your married name as well as any other names you've used.

There's a checklist of documents you need to send below the application link on the USCIS website.

Make sure you take all marriage and divorce certificates with you to the interview as well as your birth certificate.

You'll also need your state-issued ID, passports you've had since becoming a permanent resident.

Also a good idea to take any court documents that relate to offences (proof you've paid fines, etc.).
