Cher Monsieur ou Madame,

Nous accusons réception d’une pétition en provenance des services américains d’immigration et de naturalisation portant sur votre droit au statut non-immigrant K-1 se rapportant aux étrangers se rendant aux États-Unis dans le but d’épouser une personne jouissant de la nationalité américaine, en vertu de l’article 101 (a)(15)(K) du Code d’Immigration et de Nationalité – tel que modifié.

Afin de mieux vous préparer à votre entretien avec l’officier consulaire, veuillez lire attentivement et suivre les instructions ci-dessous :

D’ABORD :

1. Remplissez le formulaire électronique DS-160 : A l’aide d’un ordinateur connecté sur internet, visitez le site suivant https://ceac.state.gov/GENNIV remplissez le formulaire de demande électronique , et assurez-vous que la page de confirmation est imprimée; car vous l’apporterez le jour de votre intretien. Notez que ce formulaire est requis pour chaque demandeur de visa, par example si vous comptez voyager avec les enfants, remplissez-en un pour chaque enfant.
2. Passeport (s): Pour entrer aux Etats-Unis, chaque demandeur de visa doit être titulaire d’un passeport en cours de validité. Le passeport doit être valable d’au moins six (6) mois ou plus à compter de la date de délivrance du visa. Faites une photocopie de la page du passeport qui contient les informations biologiques.

ENSUITE : Veuillez rassembler tous les documents requis ci-dessous s’appliquant à votre cas particulier :

1. PHOTOGRAPHIES. Veuillez fournir deux photographies couleur pour vous-même et éventuellement tout enfant voyageant avec vous, dimensionnées de deux pouces carrés (50×50 mm), présentant une vue de face totale, sur fond blanc, à tête découverte.
2. ACTES DE NAISSANCE. Veuillez apporter l’acte de naissance original ainsi que deux photocopies simples pour vous-même et pour chaque enfant voyageant éventuellement avec vous.
3. ACTE DE MARIAGE. Si vous étiez précédemment marié, veuillez fournir deux photocopies de votre acte de mariage ainsi que deux photocopies de l’acte de divorce ou de décès pour prouver la nullité d’un éventuel mariage antérieur.
4. EXTRAIT DE CASIER JUDICIAIRE. Vous devrez fournir un extrait de casier judiciaire ainsi qu’une photocopie pour vous-même et tout enfant âgé de 16 ans et plus devant vous accompagner. Au Cameroun, le Tribunal de Première Instance – et non la police – est l’institution compétente en matière d’extrait de casier judiciaire. Si vous avez résidé à l’étranger pour une durée minimale d’un an, veuillez fournir un certificat judiciaire de ce pays-là.
5. PRISE EN CHARGE. Celle-ci sert à démontrer que vous ainsi que vos enfants éventuels ne courrez pas le risque de devenir une charge publique aux États-Unis. Demandez à votre fiancé(e) d’établir une déclaration de prise en charge (Affidavit of Support) à l’aide d’un formulaire I-134 (http://www.uscis.gov/files/form/I-134.pdf) auquel elle devra annexer sa déclaration la plus récente de l’impôt sur le revenu, à savoir le formulaire 1040/W2.
6. EXAMENS MÉDICAUX. Prenez des dispositions pour que ceux-ci soient faits par l’un de nos médecins agréés, et deux semaines avant la date de votre entretien avec l’officier consulaire et vous présenterez votre rapport médical dans une enveloppe scellée. Tout mineur non marié se rendant aux États-Unis avec vous doit également être soumis à ces examens.

1. PREUVES DE RELATION. Vous devrez fournir des preuves complémentaires de l’authenticité de votre relation avec le pétitionnaire. Veuillez apporter des documents tels que des photographies, des factures de téléphone présentant des appels émis depuis les États-Unis, des courriels, des lettres reçues au fil des ans (avec enveloppe portant les noms de l’expéditeur et du destinataire et les cachets postaux), ainsi que des preuves d’aide financière de la part du pétitionnaire telles que des reçus de retraits Western Union, et tout autre document susceptible de prouver de votre relation avec la pétitionnaire.

FRAIS DE DOSSIER : Les frais de dossier s’élèvent à 265 dollars pour chacun des demandeurs, à payer en espèces en dollars américains ou l’équivalent en francs CFA. Veuillez apporter vos frais le jour de l’entretien. Vous pourriez contacter l’Ambassade en appelant au (237) 2220-1603 pour vous renseigner sur le taux de change.

Vous devrez apporter le jour de votre entretien, tous les documents ci-dessous requis, ainsi que la page de confirmation du formulaire électronique.

Dear Sir/Madam,

The Consular Section has received a petition from the Immigration and Naturalization Service entitling you to a K-1 (fiancé(e)) non-immigrant visa classification as an alien proceeding to the United States to marry and American under the provisions of section 101(a)(15)(K) of the Immigration and Nationality Act – as amended. In order to prepare for your interview appointment with the Consular Officer, please read and follow carefully the instructions below.

1. Fill out the DS-160 online application available at https://ceac.state.gov/GENNIV and make sure you print out the confirmation page and bring it with you on the day of your interview. This is also required for each applicant, if you are applying with children.
2. Passport(s): Each applicant must have a valid passport to travel to the United States. The passport must have at least six months validity beyond the issuance date of the visa. Obtain a photocopy of the biodata page of the passport.
3. Photographs: Obtain two color passport-size photographs (50 x 50mm) for each applicant. The photographs must show each applicant’s full face without a head covering, against a white background.
4. Birth Certificates: Bring the original birth certificate and two photocopies for yourself and any children eligible to apply with you.
5. Marriage Certificate: If you were married previously, obtain a copy of your marriage certificate and a copy of the divorce or death certificate to prove the dissolution of each prior marriage.
6. Certificate of Non-Conviction: You must also obtain a certificate of non-conviction for you and each accompanying child over the age of 16. In Cameroon, the certificate is issued by the High Court, not by the police. If you have resided outside Cameroon continuously for a year or more, you should obtain a police report from the country of residence.
7. Evidence of Support: Have your fiancé submit an affidavit of support using Form I-134 (available at: http://www.uscis.gov/files/form/I-134.pdf). The recent yearly income tax returns (generally 1040/W2) should be attached.
8. Medical Report/Examination: Arrange for a medical examination with any of our panel physicians. The report will either be conveyed directly to the embassy or handed over to you for onward submission in a sealed envelope. A medical examination is also required for each minor child who will be applying to accompany you to the United States. We advise that you secure an appointment before going for your medicals. We also advise that you do the medicals about two to three weeks before your interview.
9. Evidence of Relationship: You have to submit secondary evidence that establishes a bona fide relationship with your petitioner (fiancé). Bring with you documents such as photographs together, emails, letters, phone records, evidence of financial support such as western union receipts, and any other items/documents to prove your relationship to the petitioner.
10. Application Fee: The application fee is $265 per applicant. The fee (or its CFA equivalent) should be paid in cash on the day of interview. You may call the embassy on (237)2220-1603 to confirm the prevailing exchange rate.
11. You should bring all of the above documents, including the confirmation sheet(s) of the DS-160 with you on the day of your interview appointment.

INSTRUCTIONS AND INFORMATION FOR MEDICAL EXAMINATION

OF IMMIGRANT VISA APPLICANTS

Before your immigrant visa interview at the U.S. Embassy, you must obtain a medical examination from an Embassy-designated physician. Two physicians are designated by the U.S. Embassy in Yaounde to perform medical examinations, and you may select either one*:

• You should contact this clinic to book an appointment for your medical examination and/or obtain information about testing procedures and fees. Please have your medical examination performed promptly (preferably at least 3 weeks before the interview date), as we cannot issue a visa without a satisfactory medical report.

NOTE: Medical examinations conducted in connection with immigrant visa applications have a maximum validity of six months. If you cannot arrive in the United States within six months from the date of the medical examination, you may need to repeat the examination.
