Hi need help.I'm confuse what should I write in my N400 form, My single name is

Last name-LOPEZ

First name-MARIA

Middle name-TORRES

My GC is

Last name - SMITH

First name - MARIA

Middle name -TORRES

I like to change it to SMITH MARIA LOPEZ that the same on my PHIL passport

My question is what should I write on my current legal name stated in Part 2 the same name on my GC ?and do I need to write Part 2 number 4 name change?

Your legal name is the name you currently use, even if it's different from the GC, so this goes on part 1. Then you put the name how it's on the GC, this is part 2. Then you put in part 3 any other names used (maiden, alias, married, etc). Then in part 4 what you would like you new name to be, this is what your naturalization certificate will say.

You confused me a bit with the Smith because that's a last name. But let's say your legal name is MARIA TORRES SMITH, this would be the name you use on all of your stuff so this goes on part 1. Your GC says MARIA TORRES SMITH, this would go on part 2.

In part 3, you put your single name MARIA TORRES LOPEZ and any other name you used before.

Does your passport say MARIA TORRES LOPEZ or SMITH? In part 4, just put what the name you want to be known by.

My passport name is MARIA LOPEZ SMITH that is the name I want to use in my citizenship so I will change my name in part 4.

Yes, so you enter that name in part 4 and your certificate will reflect that name. You will then have to update your SSN, DL, etc once you take the oath and receive your papers.
