For the I-765 would be 2 photos, the form, and I was recommended to send a copy of the I-94 and the passport bio and k1 visa page.

For the I-864 I recommend you to read the instructions of that form, it needs the IRS information, and demonstrate that the income is above the poverty level 125%. ..and there are other recommendations. Just follow the instructions here http://www.uscis.gov/i-864
