differences in DS 160 and I 134 forms for names

Hello Friends,


My Mom in law does not have family name in passport but the given name is her first and last name.

she went for finger printing today and was told to fill the DS 160 again by putting her first name as unknown and last name as the given name mentioned in passport.

Now the i 134 that I filled, has her first, last and middle name filled correctly. it is not as per the new DS 160 or the passport.


Will this create an issue? Or shall i just ask her to take the 1st page print out of I-134 and update the name just the way it is in the new DS 160? Since, the second page of I 134 is where I have signed.


please advise and consider this urgent.
