Not sure this is the right form, but it was the best I could see.

My wife entered the USA April 24, 2013, and married 6-22-13, and applied for AOS, and 2 year GC by the end of 2013.

But her Philippines passport was expiring, August 2014, and we needed to get it re-newed, before it ran out, to avoid going to Wash. DC for 3 days.

There is a program called Philippine Outreach Program, that travels around the South East, and does documents to keep you from going to the embassy in DC.

WE drove from central South Carolina, to Augusta Ga., after getting an appointment, to re-new her passport.

DO NOT BELIEVE WHAT IS ON THE WEB-SITE WHEN MAKING APPOINTMENTS, as the cost is not 60.00 cash or money order, it is at least 91.00, or more!

ALSO, the web-site says, you need only 4 documents, your old passport, a copy of the front page, your ROM, and the application for passport re-newal.,.,.,.,THIS IS NOT TRUE!!!!!!!!!!!!!!!!!!!!! Luckily, I decided to take every document we had, certified marriage license, NSO birth cert..,.,..,.and many more!

If WE had not taken every thing we had, a 12 hour trip would have been wasted.,.,.,.,do not believe what you read on the web site.,.,.it is completly different.,.,.,the Outreach personnel.,.,were very nice, professional..,.and very nice to us.,.,.,but was in shock, what all they asked for.,.,they kept so many of the documents we will have to replace!

Now, wait 6-8 weeks for the passport.,.,.,and was only 21.00 to mail it to us.,.,.,.what a deal! Great staff and workers,.,.,.,.VERY BADLY INFORMED ON THE WEB-SITE, OF WHAT IS NEEDED, AND THE COST!
