I am assuming your fiance will be familiar with the Indonesian terms, so here goes:

#1 She needs to go to her local Polres first, to get Surat Keterangan Catatan Polisi (SKCK). In order to get this, she needs: letter from kelurahan, 4 passport photographs (4cmx6cm), photocopies of KTP and KK, and be prepared to pay "administration fees." I had to pay 25000 IDR for mine (in Jakarta). They will also take her fingerprints. Tell the officers at Polres that she needs SKCK in English, they will refer her to Mabes Polri at Jalan Trunojoyo, Jakarta Selatan.

After getting SKCK from local Polres, she needs to bring that to Mabes Polri, along with more photocopies of KTP, KK, passport, birth certificate, 3 photographs, and that little card she will get after the fingerprinting. Interestingly enough, Mabes Polri does not ask for "administration fees"

Mabes Polri will take the original SKCK and give her another one in English. That's the one you need to present to the embassy.

#2 Not sure what this is for, we've only just started the process ourselves.

Keep us posted on the progress and good luck!

I received a PM asking the details of the process. I thought I would share my answer here instead so anyone else who needs it can read it too.

First, go to your local kelurahan, ask them to issue a letter for the police station (polres/polda). For this you need photocopies of KK (family registration card) and KTP (ID card). Time required: 2 days? Depending when the officials are in and perhaps other factors.

Once you have that letter from kelurahan, go to the police station (Polres/polda). Ask them for SKCK (the police certificate) and mention that you need this SKCK to obtain another SKCK in English, which is only issued by Mabes Polri. For this part you need the original letter from kelurahan, photocopies of KK and KTP, and 4 photographs. You will also need to fill in a form there. Questions asked on the form: personal details, parents' name, education history, employment history, have you ever been abroad, where you were when the communist rebellions happened (I guess for most of us the answer is: unborn, learnt it from school), and a long list of questions about you facial features (shape of nose, ears, eyes etc... just answer normal to all that). Time required: 1 day. Go there early (before 10am) and you should be able to go home a couple of hours later with the SKCK.

When you have the SKCK from polres/polda, it should say that the purpose of its issuance is to obtain SKCK in English from Mabes Polri. So now you are able to go to Mabes Polri in Jakarta (Address: Jalan Trunojoyo, Kebayoran Baru) to get your English police certificate. Bring the police certificate in Indonesian (SKCK), photocopies of KK, KTP, birth certificate and passport, and 3 photographs. Time required: 2 days. Bring it in one day, they will tell you to pick it up the next day.
