Looking for advice on a couple of things on this form.

First, my Fiancee will be entering the US with our child who is a US citizen with a US passport. Since I have already sworn in declaration to support my child, does she go under "Name of spouse and children accompanying or following to join person: " or "The following persons are dependent upon me for support" I felt it was the latter.
Also it asks about previous affidavits of support. One was filled out in 2003, which the 10 years have already passed. Should it go on here anyway?
