I had my cousin fill out the DS-260 and I'm reviewing over the information he entered. On the page for "Family Information: Parents", for his mother he put her first and last name that she was born with. I figured that they wanted the current name of the parents, as opposed to the name on their birth certificate.

The real question I have though is, if they are looking for the current name of his mother, do I include her middle name under given name? For many women in the Philippines the middle name is dropped when they get married, and the surname at birth becomes the new middle name. So if Mary Michelle Smith marries John Williams her name becomes Mary Smith Williams.

Typically I would think to include a middle name when writing a given name, as I figure most women keep the same first/middle when getting married, but that is not the case with my aunt. Ultimately I doubt it'll confuse the NVC as she is the primary beneficiary and my cousin is a derivative, but I would like to make this as correct as possible. Thanks for any insight you can provide.
